Brief Answer:
complaints suggestive of depression

Detailed Answer:
dear,
Welcome to HCM
I an understand the amount of distress that you are going through at present.

After going through the description of complaints you have presented here, I could gather following information
1) persistence sense of worry and restlessness.
2) persistent bad mood.
3) Difficullty in maintaining concentration, amounting to memory problems.
4) somatic complaints of headache, general bodyache.
5) decreased energy levels, extreme tiredness.
6) reduced interest in routine activities.
7) sleep disturbances.

well, these symptoms are suggestive of depression (severe). They have caused significant impairment in your functioning.

There are few symptoms suggestive of panic attacks, that is abrupt onset of fear associated with palpitations, dizziness and tremors.

I could guess few stressors to be present such as
con current Medical issues, etc.

my questions are
1) have you ever been reffered to a psychiatrist in past?
2) since how long have you been experiencing these symptoms?
3) are there thoughts about ending your life coming to your mind?
4) Are you consuming any substance, alcohol? are you on any medications?

answer to these will help me have a better idea.

Considering all this,
I would suggest
1) Good part is your condition is curable and with the help of regular medications and follow up you can lead to a normal life like others.
2) not so good part is that the medications needs to be taken for bit long duration may be for year long.
3) I suggest you to see a local psychiatrist soon as possible. He will run you through certain battery of verbal questions and gather elaborative history on the above complaints. depending he will start with some medications.

you can discuss your queries here in future.
