Question: once every six or seven days my esophagus will gradually begin to fill with fluid - my nose and throat will become congested - in time I will start gagging and spitting trying to get rid of the fluid - this will last for a couple of hours - the next day I am very tired and my chest is sore - I will feel very weak - by the end of the day I will be better and stay that way for the next six or so days - then it will come back in another six or so days

The symptoms you are describing is likely due to Gastroesophageal Reflux disease (GERD). GERD is the reflux of acid in the stomach to the esophagus and up the throat.

GERD is seen after Heller's myotomy. During myotomy the lower esophageal sphincter is cut, thereby making it easier for food to pass down, but also gastric acid to regurgitate back into the esophagus.

You can try few life style changes to help with GERD-

-Avoid fatty, fried, spicy food, chocolate, and carbonated drinks
-Limit caffeine intake
-Avoid alcohol and smoking(if at all do)
-Reduce weight, if needed
-Give at least 2 hours gap between dinner and bed time.
-Eat small but frequent meals
-Raise the head end of the bed by using wooden blocks under the bed post.
-Following low-fat dairy, diet seems to help some people.
-You can also take PPI/antacid like Pantoprazole 40 mg every day in the morning before breakfast.
