Suggest Treatment For GERD And H. Pyroli Infection

Question: Hello Sir/Ma'am.
I have been diagnosed with GERD and H pyroli Infection. Since past 4 month i am on medication of Adiza 10 mg ( ilaprazole tablet, once daily) and domeperidone 10 mg ( twice). My symptoms are as follows.
1. Breathing Issues- I feel heavy breathing. ( episode occurs almost on the daily basis)
2. Sometimes I feel tightness in the chest during the issue of heavy breathing
3. at time i feel difficulty in swallowing saliva ( episode is less frequently)
4. no heartburn or bloaty feeling or fullness of stomach.
the intensity of chocking is less but it is not completely recovering. I had chest x ray endoscopy and other allergy tests which are normal. I Have consulted a pulmonolist and they are of the opinion that it is not a respiratory disease.
My queries are:
1. what is cause of the breathing issue and why it is not getting cured even after 4 months of regular medication.
2. what is your advice.

Dear Ms XXXXXXX

GERD is known to cause Hyper reactivity of airway. It occurs due to occasional aspiration of acidic stomach contents into the airway. This leads to spasm of smaller airway causing the type of Respiratory symptoms you are facing. Associated stress and anxiety could be predisposing factors.

Apart from medication, Gerd requires lifestyle modifications for adequate control of symptoms. Even though you presently have no typical symptoms of Gerd like heartburn or regurgitation, it could still be present.

I would suggest you the following investigation in consultation with Pulmonolgist

1. Pulmonary function test
2. HRCT to rule out small airway disease

In the meantime I would suggest you the following lifestyle modifications

1. Eat small quantity of meals more frequently.
2. Avoid sleeping or lying down immediately after consuming food or liquid
3. Include high fibre diet
4. Avoid fruit juices / carbonated beverages
5. Steam inhalation : Preferably using electronic steamer. 15 to 20 minutes each session twice a day. It will help you with your respiratory symptoms
6. Bedtime warm saline gargling : To a glass of lukewarm water add 2 spoons of common table salt. Gargle the throat with it
