Suggest Treatment For Acanthosis Nigricans Infection

Question: Hi,

I have a problem of dark velvety ring around neck and under arms (probably Acanthosis Nigricans). I want to get rid of this problem.

From 27th March'15 i am using Kojivit cream , as prescribed by Dr. XXXXXXX XXXXXXX on Health Care Magic and i am not seeing any improvement.
My Height is 5'.2" and weight is 57

Please Help.

Hello. Thank you for writing to us.

I have taken note of your query and I have reviewed the Image. Acanthosis nigricans is a difficult condition to treat.

This thick, velvetty texture of skin in acanthosis nigricans is supposed to develop because of underlying insulin resistance. High insulin levels in these individuals induce skin changes of acanthosis nigricans.

-Rarely acanthosis can be familial. Is there a family history?
-I also suggest you to get your fasting blood sugar levels checked? this is because acanthosis is more common in type 2 diabetes which is associated with insulin resistance.
-Do you have any underlying hormonal abnormality e.g irregular menstural cycles, coarse facial hair, acne Or PCOD?

I would suggest that you be started on tablet metformin 500 mg twice daily. Metformin inproves insulin resistance and therefore brings down insulin levels and thus benefits acanthosis.
Topically, instead of kojic acid, you may use tretinoin 0.025% cream, once daily at night.
