Brief Answer:
Fluid removal with needle depends on quantity.

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Your sister suffering from endometrial cancer stage IV A is a concern.

Having worked as radiologist in a cancer institute for few years, I have seen many patients having problems similar to that of your sister. In stage 4a the objective of treatment is to reduce spread and pain from the cancer.

The Chest X ray report shows that there might be mild inflammation of lungs. Most people having this finding on chest are almost normal and not something to worry about. It can be due to mild cough and side effects of earlier chemotherapy or radiation. It is just an observation on X ray and if she is having cough then they can give her symptomatic treatment.

The fluid in ultrasound scan needs to be quantified and then if the quantity is more than about 3 - 5 litres then it can be drained out using tube and needle placed in the abdomen. If it is less than 500 cc then it will be absorbed with time. Radiation therapy is only for stopping the cancer from spreading further and to control local complications of spread like tumor invasion.

Cancer patients need special care during chemotherapy and radiation and this can help avoid infections. I suggest that any infection should be controlled in early stages. Please find out if she has any breathing difficulty and get emergency treatment if required.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Brief Answer:
Pls discuss with scan and blood reports attached.

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.

As there was fluid collection in abdomen and 3.5 litres water was taken out, it is a sign of advanced disease.

It is important to see all reports including CT scan, blood reports, liver and kidney function tests before discussing on her condition and future course of treatment.

By saying 2 to 4 months your doctor might be giving an approximated average life expectancy after knowing the present condition of your sister. However, some patients can get lucky and stay for 6 months or more even in last stage.

It is important to know why she is unable to eat and this can be known only after clinical examination and speaking with the patient as you say that she is feeling hungry but unable to swallow foods. Please give her lots of juices and soft diet for energy.

Please let me know in details what reports have come ok and within normal limits.

Please attach your reports again else mail them to YYYY@YYYY with subject as ATTN: Dr Vivek Chail with query Id 148325 and I will be happy to reply at the earliest.

Hope your query is answered.

Brief Answer:
Uploading CD to Dropbox is the best option.

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.

Welcome and you can always upload the CD containing images to a file sharing like Dropbox and send me the link.

Uploading CD and reports is the best options I suggest. I have helped many patients that way. They upload the CD and scanned reports and I download and view then in detail.

Courier facility is not available and I am sorry for that.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
