Suggest The Ideal Dosage For Insulin Taken For Diabetes

Question: Hi, I am taking Insugen 30-70 16 units twice a day and Istamet 50/500 in morning. But my sugar levels are not in control. Earlier i was taking Insugen-N and glycored plain .30-70. Should i increase the number of units.Am i taking the right medicines.
My fasting is 90-120 and before my dinner my sugar is aro

Thanks for your question.
Noted your concern.

As you are oral medications along with insulin there likely Type 2 diabetes in your case.
Though possibility of other forms of diabetes need to be considered at this age.
Detailed history taking & physical examination help in confirming the type.

Following factors play important role in planing for treatment in such a cases
- Your Body Mass Index.
- Family history of diabetes.
- Current blood sugar control.
- Planning for pregnancy.

Considering Type 2 diabetes I may suggest.
To increase Tab Istamet 50/500 twice a day.
May add Tab containing Metformin 500 mg once or twice a day depending upon physical examination.
May advise to continue Insulin as it is.
With close monitoring of blood sugar levels with home glucometer as follow-
- Fasting
- 2 Hours after breakfast
- Before lunch
- 2 Hours after lunch
- Before dinner
- 2 Hours after dinner
- Occasionally at 3 am.
Any 2 to 3 readings from above daily.
Additional readings if symptoms of hypoglycemia.

Assessing blood sugar level chart weekly one can make appropriate changes in medications.

Along with this diabetic diet and regular exercise are important.
