Brief Answer:
Two different antibiotics given simultaneously.

Detailed Answer:
Hello XXXX and welcome to HCM.
As an Urologist, i can understand your concern.

You've been given two antibiotics, Azogen and Zertalin.
Azogen is Nalidixic acid, which's given in dose of 5oomg.

Zertalin is another antibiotic, Azithromycin, given with it in 500mg dose.

If you want to think of combined total dose of antibiotic as 1gram of antibiotics, that's your interpretation.
As doctors, we don't use that interpretation.
It's just combining, two different antibiotics, for your treatment.
You're welcome to ask any other doubts, you may have.

Wish you good health.

Thank you. My concern is, if I take both simultaneously, which is a total of 1,000 mg., is that a toxic dose? I was diagnosed on 2/23 and suffering a few days prior so I'm very eager to get rid of this once and for all. Just don't have proper dosage info since I am in Mexico and one doesn't get inserts or dosage instructions with meds here.

Brief Answer:
combination antibiotic therapy is standard

Detailed Answer:
Hi again,

Combinations of antibiotics are routinely used, all over the world.
If there's indication for it, certainly it's given.
This is based on your urine culture report, after the first course of antibiotic.

Drink 10-12 glasses fluids daily.

Wish you well.

What is the proper dosage for each med? Pharmacist mentioned 1x per day re the antibiotic, but since the fenazopiridina also has antibiotic, should I take each pill once per day? Would like to take the one with the fenazopiridina more often as I am still experiencing frequent discomfort. I will up my water intake as directed.

Should the two pills be taken at the same time of the day?

Did not get a second culture; just threw myself on the mercy of the pharmacist, showed him all of my paperwork/prescriptions and advised the Cefuracet had not worked.

Thank you Dr. Mangat.

Brief Answer:
Always see a doctor first.

Detailed Answer:
Hi again,

Your situation has put many people, in a similar dilemma.
A pharmacist isn't a doctor.

First step in treatment of any ailment is always, see a doctor.
If you've urinary infection, see an Urologist.
Treatment is considered after proper examination and testing.

1.Whether one antibiotic or two is needed, is for the doctor to decide.
Also, the dosage depends on your symptoms.

2. Two pills can be taken at the same time.

3. An Urologist would never start a second course of antibiotic, without an indication for it, after checking the urine culture.

Wish you well.

I can tell by my symptoms that more antibiotic is indicated. I get UTIs about 3 times per year so I am familiar with what a cure feels like and I'm not even close. The doctor did not want to give me the one that has worked for me in the past, Bactrim, and instead prescribed Cipro. I will not go back to him as he is an ER doc, not a urologist. If this new course of meds does not work I will of course seek the advice of a urologist. But that is exactly why I chose a urologist for this inquiry. I am in Mexico and away from my regular doctors so I hoped you would help with this.

You did not answer my question about dosage for each pill. "Two pills can be taken at the same time" does not give me the information I need.

I looked up Azogen and it indicates an initial dosage of 1 gram 4 times per day, for a week or two. So the question is, how do I properly administer the Azogen and Zertalin. The pharmacist indicated the Zertalin should be taken once per day.

I am very aware that I'm flying by the seat of my pants here, but again, I'm traveling and doing the best I can. I consider this a serious matter doctor, given the risk of kidney infection, so please answer the question or I will request a woman doctor. I did pay extra money for a specialist, and got one who clearly wants to remain obtuse and unhelpful.

Brief Answer:
recurrent UTI needs further evaluation.

Detailed Answer:
Hello XXXX,

The need for an antibiotic is determined, after examination and doing certain tests, especially, if you've had several UTI's in a year(recurrent UTI).

The tests advised are the following :
1. urine routine and culture.
2. blood routine, creatinine, RBS, platelets, LFT,VDRL,HbsAg,and HIV.
3. ultrasound scan-KUB,with residual urine, and TRUS.

According to the reports and local examination, proper treatment is advised.
If UTI is found, then need for antibiotic and it's dosage is determined.
Whether injection or oral antibiotics, are also decided according to symptoms.
Similarly, one or two antibiotics ,and dosage, depends on symptoms.

If you can send the reports,it's easy to be able to give an expert opinion.
If you've no fever, take only one antibiotic.
Take Phenazopyridium tablets twice or thrice daily , for burning sensation in urine.

Hope your doubts are cleared.

Wish you well.
