Suggest Tests To Diagnose Seasonal Or Viral Fever In An Infant

Posted on Mon, 13 Jul 2015
Question: Hi Doctor,
My 13 months baby (weight 16Kgs) was suffering from cold for last 7-8 days, however after administering T-mimic and Ambrodil she was recovered last Friday. (19th May)
However from yesterday night , 2 am all on a sudden she got a very high fever (103 deg ) and she vomited once.
We gave her Crocin DS (4ml) at 3 am and again she again vomited again at 3:30 am.
Her temperature was increasing again in the morning so we gave her another dose of Crocin at morning 7 am.
We have visited the local doctor and he prescribe Meftal P (6ml) at 8 hours interval and T-mimic. (prescription attached)
My questions are as below
1) Is there any way to understand if this is a seasonal or viral fever?
2) After giving Meftal P if her fever is back within 4-5 hours can we repeat the dose again or we need to wait for 8 hours? And is the dose of 6 ml is correct?
3) She is not having any mucus congestion now , then is it required to give T-mimic?
4) Is meftal P ok? or we need to give her Crocin DS? She is crying a lot for fever.

Brief Answer:
You can give meftal p

Detailed Answer:
Hi
Thanks for writing .
1. If your baby is active and playful and passing urine well it could be mostly a viral infection. But because of the fact that she had fever last week, it could also be other fevers like typhoid etc. Your pediatrician has examined and found that she is normal clinically.
I would suggest you to follow his advice and recheck again in a few days if fever doesn't settle. Blood tests may be required if fever doesn't settle in next 2 days.
2. No wait for 8 hours for meftal p. You can give crocin if fever comes back in 4 hours.
3. T minic may not be required if no symptoms.
4. Yes meftal is ok

Wishing your child good health and speedy recovery.

Hi Doctor,

Thanks for your advice.
However just wanted to inform that my baby was not having any fever last week but she was suffering from cold only (not cough).
from yesterday night only she is having fever.

So for viral infection to settle down do we need to give 2 days or 3 days? Based on that we can decide on the further examinations.

Also wanted to know can we continue both crocin and Meftal together?
like at 8 am if I give her meftal the next dose will be at 4 pm (after 8 hours) , in the mean time if fever comes can I administer Crocin at 12 pm (after 4 hours) and again give meftal at 4 pm?

Thanks and Regards

Brief Answer:
you are correct

Detailed Answer:

That is good. If no fever in the previous week you can wait for 2 to 3 days. No need to worry.

If fever doesn't settle down by then or if she becomes dull with poor intake then you need to follow up again.

Yes, you can give crocin as you mentioned and then give meftal at 4 pm.

Thanks a lot doctor.
As her weight is 16 kgs, can you please confirm if 6ml of meftal P is adequate for her. Also please let me know the dose of crocin ds , if we need to give it in between?

Hello Doctor,

Thanks for your reply.
We are giving her Meftal P at 8-9 hours gap.
But from yesterday night she has developed some caugh and also vomited thrice at a 3 hours interval.

Just wanted to understand is there any kind of side effect of Meftal P which is causing this or vomiting ( as we are applying this thrice a day) or this can be due to some other reason and we need to check some doctor again?

Thanks and regards

Brief Answer:
Vomiting can occur with meftal p

Detailed Answer:
Vomiting can occur with meftal p.
It can be the reason for your child's vomiting.
You can stop meftal p and give crocin Ds alone for fever.
If vomiting persists then consult your doctor to rule out infection.

Thank you for your reply.

My baby's fever has subsided but she is coughing a lot right now. Specially while eating, she is coughing and as a result vomitting most of the food.

Can we give her some safe medicine right now for her cough?

We can take her to doctor only tomorrow morning, but if you can suggest some immediate medicine, it will be very helpful.

Thanks

She is not even willing to have any semi solid food too. Can it be that she has throat ache? Or can there be any other reason?
doctor
Answered by Dr. Arun (21 minutes later)
Brief Answer:
possible

Detailed Answer:
Yes throat infection is possible but needs confirmation by clinical examination.
Crocin Ds should help in decreasing pain.
If fever has settled then there is very less chances of serious illness apart from throat infection or mouth ulcers.
Your pediatrician should be able to confirm when you visit him.
Meanwhile continue liquids if Semisolids are not being taken.
