Suggest Tests To Diagnose Seasonal Or Viral Fever In An Infant

Question: Hi Doctor,
My 13 months baby (weight 16Kgs) was suffering from cold for last 7-8 days, however after administering T-mimic and Ambrodil she was recovered last Friday. (19th May)
However from yesterday night , 2 am all on a sudden she got a very high fever (103 deg ) and she vomited once.
We gave her Crocin DS (4ml) at 3 am and again she again vomited again at 3:30 am.
Her temperature was increasing again in the morning so we gave her another dose of Crocin at morning 7 am.
We have visited the local doctor and he prescribe Meftal P (6ml) at 8 hours interval and T-mimic. (prescription attached)
My questions are as below
1) Is there any way to understand if this is a seasonal or viral fever?
2) After giving Meftal P if her fever is back within 4-5 hours can we repeat the dose again or we need to wait for 8 hours? And is the dose of 6 ml is correct?
3) She is not having any mucus congestion now , then is it required to give T-mimic?
4) Is meftal P ok? or we need to give her Crocin DS? My baby is crying a lot as she is not able to sleep due to fever.

Most of the fevers are seasonal viral infections.

Detailed Answer:
Thanks for asking on HealthcareMagic as well as for choosing me specifically to answer your query. I would try to address the issues one by one:
* Usually the activity of the child remains fine in case of a viral infection. In other words, except for during the period of high rise of temperature the child remains active and fine. If blood tests are done, the reports are more or less normal. It is only if the neutrophil count is raised that you should suspect a bacterial cause. Otherwise, it can more or less be assumed that the fever is viral. Usually viral infections are associated with cough and cold.
* Meftal P contains Mefenamic acid the dose of which is 4-7 mg/kg/dose. For a child of 16 kg, the dose should be about 80 mg. The syrup contains 50 mg/ 5 ml. So, 8 ml would be the right amount to be given to your child. 6 ml is a little inadequate. If you require to give fever medication within 8 hrs of the previous dose, it is better to give paracetamol (Crocin DS) rather than giving meftal P earlier than 8 hrs.
* T-minic can be given. But I would prefer giving steam inhalation. It works better. Just sit with the child on your lap and place the source of steam before you. The child will automatically breathe in the water vapor. It will help in diluting the mucus.
* Meftal P is fine to be given. If you require to repeat a dose in less than 8 hours, use Crocin DS. Fever needs to be brought down at any cost. Try tepid sponging.

Can you also let me know the dose of crocin ds , if we need to give it in between?

Also how long do we need to wait to see if there is any improvement before going for any further blood test? As our local physician has not suggested for any tests as of now..

And any requirement of taking antibiotics as of now?

If she has fever in less than 8 hours from dose of Meftal P, give Crocin DS

Detailed Answer:
Thanks for writing back. Crocin DS Suspension contains Paracetamol 240 mg/5ml. The dose of paracetamol is 15 mg/kg/dose. For your child of 16 kg, it should be 240 mg. So, 5 ml of the syrup needs to be given per dose. The minimal interval between two doses of paracetamol is 3-4 hours. There is no relation between the doses of paracetamol and meftal p.

Antibiotics are ineffective in viral infections and so they are not needed as of now. Unless the child becomes lethargic, I do not think there is any reason to worry and you need not undertake investigations till then. However, in case the child becomes lethargic, you should rush her to a doctor without delay.

one more doubt i have given her 5 ml dose of meftal P at 1 pm , so time for next dose is 9 pm.
Bur I just checked that she is not having temperature now.. So should I still give her another dose or wait for some more time and then give the second dose.

My concern is without temperature if I give her a dose that might impact adversely.

Another point though very minor is sometimes she is also coughing (once in an hr and so) for this do we need to give any medicine like T-mimic or Ambrodil or anything else?

Please do not give Meftal P regularly on basis of time. Give it only if there is fever. If not, skip the dose. Since she does not have temperature now, no need to give any dose at the moment.

Giving her dose without her having temperature would not affect her adversely but would create unnecessary load on the liver to detoxify. Moreover, frequent unnecessary doses may lead to gastritis. So avoidance of unnecessary dosing is advisable.

Rather than giving T-minic, I would suggest giving cetirizine syrup for a few days. Otherwise nothing else is needed.

Hello Doctor,

Thanks for your reply.
We are giving her Meftal P at 8-9 hours gap.
But from yesterday night she has developed some caugh and also vomited thrice at a 3 hours interval.

Just wanted to understand is there any kind of side effect of Meftal P which is causing this or vomiting ( as we are applying this thrice a day) or this can be due to some other reason and we need to check some doctor again?

You need not give Meftal P every 8 hours or so. Give the next dose only if she has fever after 8 hours or thereafter. If not, skip the dose. Meftal is a gastric irritant and hence could be associated with the vomiting. So give it only if needed. If Crocin DS can suffice, it would be a better choice. However, vomiting could be due to other causes as well. You need not worry. Giving some antiemetic should be fine. Give steam inhalation. It would help for the cough.

Thank you for your reply.

My baby's fever has subsided but she is coughing a lot right now. Specially while eating, she is coughing and as a result vomitting most of the food.

Can we give her some safe medicine right now for her cough?

We can take her to doctor only tomorrow morning, but if you can suggest some immediate medicine, it will be very helpful.

Thanks for writing back. Its good to know that the fever has subsided. I have already suggested cetirizine syrup and steam inhalation for cough. Try those for a few days and she would get relief. Cetirizine and other antiallergics have a side effective of increased sleep but that is harmless and nothing to worry about. Please do not panic. The total episode would last for a week or so irrespective of what you do.

However before getting your response by mistake my husband gave my daughter mucolite drops 1ml dose.

Now can i give her cetrizine or it will have any adverse effect?

No problems if mucolite as well as cetirizine are simultaneously given. You can give cetirizine after the dose of mucolite. You need not worry.
