Hope you will be fine dr.
as you know my case history , i have to discus somethings with you. first i told you about my weight which i have discused with you and you told me that if i have weight loss then it is serious problem. i will tell you a little detail. when my stomach start disturbing in start of sept 2014 near about 9 month before. suddenly i lost near about 3 kg. from 64 to 61 kg. and at that time i discused with you and you told me that it might be due to stomach problem , which already i have discused with you in details like ibs etc. after next 4 month in december i again check my weight , i was having 64 again. after that i i did not checked my wieght. ans as you know 3 month before doctor told me to do some tests about sugar level and my sugar was high. and i left all sweet things and i have started more walking . and i was feeling i have now lost too much weight. and last week i checked my weight it was again near abut 60 kg and half . and it give me a lot of tension. kindly answer my questions , does leaving sweat things like sugar in tea , juices , soft drinks can cause such weight loss? and i am walking more also not only because i am doing exercise becasue now a days i am working such activity to move more then normal. but the problem that i think is i am eating food in normal quantity as before but still i lost weight. kindly tell me is it possible? because things comes again in my mind about hiv and diabetes froms last one month i was very much busy and i was very less thinking about diseases but still i have some questions.
one more question that i have is, last week i was feeling tiredness in my jaws and mouth. it was not pain it was like stress or the same feeling if someone speak a lot and then get a tiredness. after 3 days of such feeling , i left the medicine "cobal Mecobalamin and after leaving i am ok now. is that medicine was the reason?
and one more thing is i am still having muscle twitchings . it not very much but these can b start at any time , or any where. but the reason i dont know. i can feel these twitchings sometimes in arms.

Hope you are feeling better ,besides your weight problem.
I am sorry for the delay but only now I could read your query.I apologize for that.

I read carefully your query and I don't think you have any major problem with your weight.It is normal that being on diet ,increasing your physical activity and stress can cause some weight loss.A weight loss of 2-3 kg in few months is not considered a significant loss and it doesn't indicate a specific condition that is causing the weight loss.
You may need to increase your calorie intake (eat a well balanced diet).Make sure to eat enough proteins a day (lean meat,chicken meat ,fish).
Remember that being continuously on stress and being worried all the time for your health,or working too hard can cause weight loss.
In your case the weight loss is gradual,small and it doesn't indicate for sure a problem.

You may need to get your fasting blood sugar checked once in 3-4 months just to make sure it stays in normal levels.
Getting a HIV test done will relieve some anxiety in you but this is your decision.

The feeling of tiredness in jaw can be related with a dental problem like teeth grinding or might be due to TMJ disorder (inflammation of the temporo -mandibular joint or jaw joint) that can cause tiredness of face muscles and pain when you open the mouth.Mecobalamin usually doesn't cause such symptoms.
This might be related to stress. The body often reacts to stress by clenching or grinding the teeth and this can makes the jaw muscles work harder.
It doesn't seem to be of concern.

Regarding your muscle twitching ,the fact that it hasn't progressed in all these months,it happens from time to time, you don't have any other symptom like muscle weakness, numbness, tingling etc, may indicate that you have just a benign muscle fasciculations that are felt like muscle twitching.They happen without any known cause and you may need to learn to ignore them.

as you know my doctor previously tested my blood sugar only random blood sugar. but sometimes i think that my wieght loss could be a possible reason that i have developed diabeties. i know i dont have a major wieght loss but i feel that i my skinny as compared to before. but i dont have the other symptoms of diabetie like frequent urine and more appetite , i have normal hunger . just because of my job i sometimes quite lunch or breakfast. that effect my stomach result in ibs . as you know. so you still think i dont have diabeties considering my first fasting blood sugar level was 107 and hba1c was 6.9?

No,I don't think you have Diabetes.You blood sugar was a bit high but not to be diagnosed as diabetes.Diabetes is diagnosed when you have a fasting blood sugar over 126 mg/dl ,or a random blood sugar over 200 mg/dl.
Since it has been 3 months since you did the HbA1c ,I would suggest to repeat the test again and repeat the fasting blood sugar to see their level now.

You should try to eat regularly and frequent meals.Try not to skip meals and make sure to take the necessary calories every day.
