Brief Answer:
Further investigations to get the proper diagnosis..

Detailed Answer:
Hi.
Thanks for your query and the history you have provided for your friend.

The most probable causes of swelling of the abdomen to look like 9 months pregnant with discomfort, legs started to swell, both Endoscopies done, yet the cause is not found as per your history, treatment has not helped her can be due to the following reasons.
-Ascites- collection of fluid into the intra-abdominal cavity.
-Very large ovarian cyst / cancer
-any other rare sort of cysts or tumor
-Gaseous distension due to obstruction of the intestine.

The causative agent causes compression on the venous return from the legs to the heart. This may be the reason of swelling over the legs.

I would advise her the following:
-X-ray of the abdomen in the standing position if possible or she can sit the chair and this will help diagnosis of intestinal obstruction.
-Ultrasonography of the abdomen and diagnosis to be confirmed by CT scan .
This will help for the proper diagnosis and plan for the treatment
-If Ascites is present, to get the fluid by tapping the abdomen and to send it for analysis.
- Investigations of the blood- CBC including the liver function tests , kidney function tests.

These reports will give the Doctors the diagnosis to decide the further investigations and plan for the treatment.

I hope this answer helps you to get an idea of your friend's illness, get an early proper diagnosis and get treated accordingly... Please feel free to ask for more if you need to or if you feel there is a gap-of-communication...
