Suggest Remedy For Solid Hard Stools In A Child With Arthrogryposis

Question: hi sir,
my son was born with arthrogryposis affecting his lower limbs...he is 9.5 months old now...weight 8.5 kgs..from the past one week he is passing solid hard stools...today we noticed bit of blood in his stools and we also noticed a cut near anal region...1)can you prescribe a lotion or cream to apply to the area so that it can heal and also 2)pls prescribe me a stool softener so that his stools become a bit loose and make it easy for him to pass..3) and also natural things that we can add to his diet so that will make the formation of stool a bit soft and easier to pass...i'll be highly thankful to you if u can help me out on this...he is having these problem after we started him on cows milk i guess..please take time to answer all the queries i have posted.

Thanks for the query

Solid hard stool causing injury to anal area should be managed carefully by stool softener agent,diet modification and local application of ointment.

You can apply gel containing lignocaine(local anesthetic) and antibiotic ointment(neomycin) for pain relief and healing of the lesion.

Keep the area dry by frequent change of diaper.Cleanse the area gently and avoid rubbing.

Give Polyethylene glycol 3350 sachet(17 gm) 1/2 XXXXXXX a day mixed with fruit juice or you can use Lactulose syrup 10 ml at bed time as a stool softener.You can give this for 3-4 days depending on the improvement in symptom.
Please get it prescribed by a pediatrician as these are prescription medicine for this age group.

Give plenty of fluids,add sugar to the feeds.Giving juice will also help in making the bowel pattern regular.Now you can give semisolid food to the baby(avoid complete smashed food).
Avoid using "cow milk" as it causes constipation due to hard casein curd.It can also cause cow milk protein allergy below one year of age.

From the medication history I got to know that you are giving tonoferon. Tonoferon contains iron and constipation is a known side effect of iron supplements.So I suggest you to recheck the dose of tonoferon you are giving and to stop this for few days(or decrease the dose).

sir his paed prescribed him milk of magnesia 3.5 ml daily for 7 days and annovate cream...is it good or i shall switch to your meds?please let me knw if the medicines prescribed by his paed have any side effect or dosage is inncorrect

The Milk of magnesia(Magnesium hydroxide) is also a laxative and can be given in a dose of 0.5 ml/kg in this age.So for your baby 3.5-4.5 ml/day is sufficient.
Avoid using the milk of magnesia for long term(more than a week) or repeatedly(without prescription) as excessive intake can cause magnesium toxicity.

Annovate contains lignocaine(local anesthetic).phenylephrine(spasm of blood vessel) and betamethasone(steroid).
It is used mainly in case of piles/hemorrhoids.
Although you can use it for short term but I won't suggest the use of steroid and phenylephrine in this condition.
Application of simple lignocaine(2%)gel and antibiotic(neosporin)ointment will do.
