Question: I had two flat shiny red spots on the head of my penis..for about two months. ...yesterday in the shower I masturbated and apon orgasm had an excruciating pain from the rim of the head to the tip..and the spots were then raised ..and kind of sore...like an irritation. .

there are many possibilities for the symptoms you described. red spots on the head of the penis, irritation, pain on contact and redness can be due to many causes like
a) Herpes
b) Gonorrhea
c) Chlamydia
d) Syphilis
e) fungal infection

will you please tell whether you had history of sex? have you any other symptoms like urethral discharge? are you suffering from any other disease?

please send a recent photograph of the lesion that can help to make a diagnosis depending on which i can guide you.

You can use antifungal creams like Candid A twice a day for 1-week for which you require prescription of a doctor .
take a few preventive measures such as:
Maintain a good personal hygiene and avoid sharing personal items such as towels. avoid sex during this period .

I am 41 years old.married 3 years. .been with the same girl and one other for the last 17 yrs..have had a few other sexual partners. . No discharge or smells.. first time this has happened. .only meds im on are for high blood pressure and acid reflux. . (Losartan..protonix). Couldn't send picture from my phone (could not figure out how)..thank you for your time

i understood your query

As you have different sexual partners, there can be a chance of STD if any of your partner has STD (sexually transmitted disease). Though, the symptoms do not match with STD infection. To my mind, it's more likely to be fungal infection.

As i told you, a photograph is better to guide you.

You can apply anti fungal as advised you before. in case of STD, you may require antibiotic treatment.

Local examination is necessary for actual diagnosis and in such case , consultation with dermatologist will be helpful to you.
