Suggest Remedy For Rashes Around Anus While On Probiotics

Question: I have had a rash around my anus for about a week and a half. I first thought it was hemroids so i used hemriid cream. As the rash got worse i used apple cider vinager in a sits bath 4 or five different times. I have also been using probiotics. What is this and how do i treat it. The last few days the rash apears raw and is weeping.

I have gone through your query and I have also reviewed the image.
You seem to have developed intertrigo, which is inflammation of body folds and further exacerbated by candidal overgrowth. Haemorrhoids definitely make you more prone to intertrigo because of moisture (more so if you are also using a topical cream for haemorrhoids) which encourages candidal overgrowth.

If i was the treating doctor I would suggest you to use a cream combination of steroid and topical antifungal e.g beclomethasone dipropionate 0.25% and clotrimazole 1% cream. You may use this twice daily for 2weeks.
I would also suggest an Oral antifungal e.g fluconazole150mg once every week for 4weeks. A tablet of cetrizine 10mg once daily would help relieve associated itching and discomfort.

Can i get any of the suggested medication over the counter without a perscription?

There are some otc brands of steriod+antifungal cream and cetrizine tablet

Canesten -HC is one such combination which you may find otc. Cetrizine is otc. You may try these from a drug store near you. However, fluconazole tablet is a prescription product.

Any of these available in the united states for otc. I called my pharmacy and they did not carry those. But they did have tinactin (tolnaftate 1%) and Lotrimin Ultra (butenafine hydrochloride 1%) and monistat (miconazole Nitrate 2%). All creams. Any of these what i need or can you suggest some that are available in the united states?

Hi.

These are all plain antifungals whereas for you I would suggest a combination cream containing a topical antifungal and a topical steroid. What you can do is that you can try lotrimin ultra but i would suggest that you first mix it (1:1) with an otc topical steroid cream e.g triamcinolone acetonide 0.1% or betamethasone valerate 0.1%. Combining these would serve the purpose as a pre mixed combination of topical antifungal and topical steroid.
