Suggest Remedy For Poor Learning And Listening Capabilities And Bad Social Relationship

Question: Hi Doctor,



My name is XXXXX (Male, Age: 33) I am married and having one kid and working as IT Analyst.



I have the following problems.



Ø Bad social relationship (friends and families)

Ø Poor Learning and Listening abilities, I am very slow and unable to concentrate in my office work and it has been always criticized by office colleagues.

Ø If someone criticized, then I will fell into depression state and it will take at least 2 -3 days to overcome.

Ø Easily forgetting the things, especially in office.

Ø Poor email communication (slow in composing), checking multiple times and making error.

Ø Stammering during important meetings or telephonic conversations and forgets the things what to speak,also switching one task to another task and the same continues while speaking.

Ø No patient, Poor concentration and focusing.

Ø Always have own assuming things and have bad understanding about people.

Ø Unnecessarily leaving my task and working on others tasks which will not give any credit to me.

Ø I am always smartly fooled by my colleagues and getting anger later on them.

Ø I am getting anxiety if I am assigned with some of my new office task. Poor in smart work and always work too hard and no outputs.

Ø Since I have 6 years of work experience, still i am too weak in decision making and always seeking help from others.

Ø Poor in anger management and I can be easily emotional

Ø Often or sometimes (based on bad things around me) my heart rate is high and having unnecessary worries, and depressions.

Ø Poor in organizing and time management ( high in office task ), even if I tried to organize the things by today, automatically on the next day the things will remain unorganized.

Ø Sometimes, I am not sure what to speak, where to speak and making messy with the people and having guilty feelings.

Ø I don’t have a sense that to whom I need to trust and giving unnecessary inputs to others and which makes me in trouble.

Ø I have switched 5 companies in 6 years.



I have the most of the above problems from my childhood. But only four months back, I have read about ADHD and visited psychiatrist and started taking methylphenidate, but I have not discussed about depression or social relationship, as I thought, the medicine will cure my messy things and performance then everything will be OK.



I don’t find much difference while taking methylphenidate, it might help little on concentration but not much also no changes in other problems.



I am not sure, I have ADHD or SAD or GAD or mood disorder. Please help me to know what diagnosis / test are required to confirm and fix my problem.

I have read the details you have mentioned in query and I can understand your concern. The symptoms like poor social relationships, depressive symptoms when someone criticise you, difficulty in speaking in certain situations and then feeling guilty, stammering during important meetings, inability to speak relevant things during some important conversations etc suggest some social anxiety components. Unnecessary worries without any reason are also due to anxiety.

Other symptoms you have mentioned like poor learning in work, difficulty to focus during work, bad in social relationships, guilt feeling sometimes, multiple switch of jobs in short period of time, poor organisational skills, inability to finish tasks sometimes, forgetting things of office easily etc suggest some depressive states. But a lot of individuals with anxiety spectrum disorders develop depressive symptoms over time. Depression and anxiety are found co-morbid in most of individuals.

There are no specific investigations that can help in making a diagnosis. You should consult a psychiatrist for confirmation of diagnosis. The symptoms you have mentioned suggest anxiety spectrum disorder especially social anxiety disorder with co-morbid depression.

I'd advise you to get your thyroid function tests done to rule out hypothyroidism. Hypothyroidism is one of important cause of such symptoms. Also get your B12 levels checked. Low B12 levels results in increased risk of depressive and anxiety symptoms apart from tiredness, easy fatiguability etc symptoms.

Methylphenidate is amphetamine medicine and this can help to improve energy levels and focus but the drug cant help much in depressive and anxiety symptoms. For proper treatment of these symptoms medicines like SSRIs can prove useful. Fluoxetine, Escitalopram, Citalopram etc have great efficacy in such symptoms. These medicines if taken properly, result in improvement in symptoms in about 4-6 weeks. So please visit a psychiatrist for help and prescription of these drugs.

Also start taking B12 supplement and Omega-3-Fatty Acid soft gels. Both these supplements will help to improve your energy levels and you will feel better.

Dear Doctor,

Thanks you very much for your quick reply.

Apart from the previous issues, I also have few things, which is not discussed earlier.

     Restless Legs (calf muscle pain).
     Leg numbness, if sitting in a same place for long time (more than 45 mins).
     Irritability
     Constipation.
     Insomnia (can’t sleep immediately).
     Snoring
     Either be silent or repeated speech often on same topic will stop only if others warn or after watching others by their facial expression.
     Always be in Laptop or XXXXXXX checking out the same topic and wasting my time, not much interested in going outside.
     Not able to prioritize my things.
     Over confidence and getting into wrong things and then struggle (Like leaving my job and looking for a new one ( Even during academics, Bachelors I did Computer Science and masters I switched to Bioinformatics).
During my education, am good in biology but poor in maths, biochemistry).
Not understanding about other feeling but as I feel the others are not understanding about my feeling.
     No proper planning and getting anxiety during Last minute execution.

Diabetics (runs in my family) and I have been identified at age of six months (Not sure about the type). I have been taken insulin injection till my age of 9. But the recent Lab test (before 2 years) as well self test shows normal blood sugar level.

Earlier I have smoking and chewing tobacco habits and it has been stopped now, but rarely smokes, only if I had severe mood or depression.
But no interest in alcohol.


My mom has identified thyroid problems and she had taken medicine for long years. Anyhow she is no more (the reason is not identified from her diagnosis; she had blood clotting problem and hospitalized before her death).

My kid has not yet started speaking, he is 22 months old, but he is active, understanding few things but sometimes he is not interested to learn. Always he likes to watch YouTube videos in XXXXXXX It is very early stage to predict, however whether he required any test.


As of now, i am not residing in XXXXXXX most probably I will come to XXXXXXX for test.
Apart from Thyroid function test and Vitamin B12 level test, do I required any other test as I could not fly often?

For time being, can i consume vitamin B12 (1000 mg) and Omega3 (700 mg) along with methyphenidate (54mg) and will it have any drug interactions.

Hello thanks for asking a follow up question

The numbness and restless legs can occur due to some nutritional deficiencies also with B12 deficiency. Other symptoms you have mentioned match the clinical features of depression. The feelings that others are not understanding your feelings are also common symptoms of depression.

I'd also advise you to get your blood sugar levels and HbA1c levels checked. HbA1c levels provides an idea about long terms sugar levels. As there was history of diabetes so this should also be throughly investigated.

Get the tests which I have mentioned done, no need to go for more investigations as for now. Yes you can start with 1500 micrograms Methylcobalamine (B12) and Omega Fatty Acids without any fear. These supplements are available commonly. For Methylcobalamine you might need prescription. No these supplements will not have any serious interactions with Methylphenidate.

Your child is 22 months old only, don't try to make him learn things. He will learn by himself. If he is achieving normal milestones like walking, eating, playing etc then no need to worry. Some children have delay development of speech. Wait for some more time.
