Suggest Remedy For Persistent Abdominal Pain And Periodic Cough

Question: Hi, Im 28, male, 150 lbs, 5' 11".

Im experiencing some digestive/stomach issues that have been a bit more servere today.

I had a whole wheat bagel with egg, cheese, peppers and guacamole for breakfast. About a half hour to an hour later I had a strong tightness/pain in my stomach at the bottom of my rib cage. It lasted around 30 seconds or so. After it had passed my stomach has felt full like gas in my upper stomach with a slight pain or senstion like something was pushing slightly on something in my diaphram area.

I had gone on with my day, had a salad and chicken noodle soup for lunch. Ive felt ok but my stomach still had some slight discomfort.

Tonight h had to help move a motorcycle and upon doing so, leaning over was causing a weird fluttering feeling in my diaphram area which was also making my throat feel weird and felt an odd fluttering sensation there as well. I returned to sitting up and standing and it had stopped.

I have since then, still had the same little bit of stomach discomfort that has been ongoing. I took a gas x that helped a little but it feels like gas in my stomach is still grumbling. It causes me to cough periodically and I spit up white phlem, very little.

This has also seemed to affect my heart with bending over or standing up. It will beat hard and faster after bending and could hear it in my head in one occasion. When it feels like Im having a spasm in my diaphram area my heart will beat forcefully for a few beats as well.

I have had several ekgs in the recent past, along with an echo, and stress test. My cardiologist says my heart function is normal and healthy.

In regards to my stomach Ive only had an abdominal ultrasound so far and it had returned untemarkable. My bloodtests all came back good, liptids and diabolic etc.

Given that those have been clear I have not had an upper endo or anything else yet.

I was hoping for some insight as to what might be going on. It is a bit unnerving and I have some anxiety being unsure if it is anything serious. So far I am comfortable standing or sitting straight up with my back supported. My stomach just rumbles periodically but the coughing subsided.

Some insight would be greatly appreciated and understanding if possible.

From the detailed history you have provided, it seems like you are suffering from chronic indigestion problems with acute flaring up of problems. The symptoms you have mentioned are commonly associated with it.
It is common these days in this age group, mostly because of excess junk food intake and irregular food habits. When patients come to me with complaints such as yours, I generally advise them the following-

Diet modification- avoiding junk and fatty food, taking lots of water and dietary fibers orally, setting up a regular time to take food, taking small frequent meals throughout the day rather than taking large meals. Avoiding smoking and alcohol if any.
Exercise- regular brisk exercise enhances metabolism hence it becomes easy for the body to digest the food one takes. Brisk walking, jogging, cycling, swimming or yoga- all are effective if practiced regularly.
Medications- Cases which are not managed with lifestyle modification only, I prefer some medicine alongwith. Pantoprazole or ilaprazole with levosulpiride is a nice combination which makes bowel habits regular and enhances metabolism. Probiotics like lactobacillus preparations alongwith give good results. But one has to be patient and take it for 20-30 days for effect.

Also, routine blood tests like complete blood count, liver function test, stool culture etc can be done to rule out other causes.

Thanks i do feel a bit better today and am hoping to get more yogurt and probiotics a part of my daily routine.

I also want to ask if a fluttering sensation at the bottom of my rib cage where my diaphram is would be of any concern. If i am leaning on a bed at an odd angle I feel small contractions that happen a few times and go away with a change of position.
I also feel the sensation in the bottom of my throat/neck when that happens.

My heart evaluation a few months showed no issues and while I have had that sensation in my stomach I have felt my pulse and did not feel any change in rhythm or any kind of chest related pressure. I would be hoping it would not be likely to be related.

I would like to add I dont smoke or drink and walk ovcasionally to exercise.

Hello.
Thank you for writing back with some more information.
The symptoms you have mentioned can be expressed as some chest discomfort rather than pain. Upper abdomen fullness can also be associated as you mentioned.
As your cardiac evaluation is normal, also considering your age, you do not have to be much concerned about cardiac problems. The symptoms might mimic a cardiac problem, but in most of the cases of chest discomfort, it is because of indigestion rather than heart or lung causes.

Avoid smoking or drinking as you are already doing, keep doing exercise regularly.
You might consider taking pantoprazole-levosulpiride for few days, as it gives very good results.
