Suggest Remedy For Penile Rash

Question: Hi I developed a rash on one side of the head of my penis. This website was my first go to. I was screened for stds when the rash was present and came up clean. I've seen two doctors and was prescribed a steroid cream and anti fungal. It seems to go away and then as soon as I have any friction it comes back. Any ideas?

I have gone through your query and I have noted down your concern.

You seem to have balanitis. There can be multiple causes like infection commonly yeast/ candida, zoon's balanitis or it could be a drug induced eruption. Friction is usually not a cause for balanitis.

Need more clarity on skin lesion on arm

Yes. I did. I recieved 2 of them, one of them of glans and one of arm probably however you made no mention of skin lesions on your arm in your first query!!
Do you have any other similar skin lesions elsewhere like scalp, elbows, knww etc? Are the skin lesions itchy?

I can see minimal redness on side of glans.
Though it does'nt seem to be specific to a particular cause if I consider it alone, however if I consider both the lesions i.e on skin and glans, this sort of well defined patchy redness can be seen in various conditions like eczemas (commonly e.g psoriasis, seborrheic dermatitis, circinate balanitis of reiter's syndrome). Psoriasis and seborrheic dermatitis may relapse after steroid is stopped, however circinate balanitis of reiter's syndrome is self limiting.

A topical steroid would definitely help in all these cases. I suggest you to use a moderately potent topical steroid e.g triamcinolone acetonide 0.1% cream, twice daily. Another alternative would be to use a steroid free immunomodulator e.g tacrolimus ointment 0.03% ointment or pimecrolimus 1% cream, which is effective for both seborrheic dermatitis and psoriasis.

Thank you for you detailed answer. The lesion is new and is not itchy just ugly. And cause me concern. What would cause a lesion like this? I also recently developed a rash on my back and arms that was probably diagnosed as pityryasis. Could there be a connection...? Do I have reason to worry. These will be my last questions. Thank you for your time Dr.

Hi.

A well defined red, scaly plaque like this on arm is most likely to be seen in plaque psoriasis. Psoriasis may be the underlying reason for both skin lesion as well as patchy redness on glans. Although psoriasis is also one of the differential diagnosis of pityriasis rosea but p. rosea usually presents with numerous lesions arranged in a christmas fir tree pattern along ribs, on torso and proximal extremities. A skin biopsy from skin lesion would help establish the diagnosis.
I suggest you to apply the same topical moderately potent steroid i.e triamcinolone acetonide .1% or mometasone furoate 0.1% cream for both skin lesion as well the patches on glans.
