Suggest Remedy For Pain In Sacrum

default
Posted on Fri, 30 Jan 2015
Question: I just wrote and lost it. Can you tell me this - pain in sacrum which is where the mesh is attached, is unbearable; prolapse returned; incontinence now and only after surgery; dr said two years late for mesh to be involved but I reported issues almost immediately and could feel the pulling. I feel like a rock is in area of sacrum causing such pain I have to lie down. Is it not possible that the mesh tore away and has been doing so for some time? Doctors I've seen as of late don't know history; I know the prolapse and incontinence coincided; by the way, prolapse is about size of first - not operable; I made a big mistake, but have to move on to resolve and want to know intelligent questions to ask Pain Management doctor. I will forward report to you, but someone besides the student needs to read film which I believe you agree. Is there a connection to sigmoid resection and low abdominal pain? Also, he capped the colon; then cut a hole in side of rectum and attached there; also area of pain.
Just your thoughts knowing only Pain Management is seeing me tomorrow and he usually deals with backs, necks, etc. However, bless him for taking steps to have SPECT.
I addressed moderators and told them you are the best, most thorough, doctor I have dealt with and I take responsibility for being unprofessional in telling you Happy Birthday and making it personal. I'm sorry.

Brief Answer:
The sacral plexus could be the culprit

Detailed Answer:
Hi Thanks for writing in again
No need to apologize - you have all the right to wish and bless
I have a feeling there is something unusual with the sacral nerves and the placement of mesh
The pain consultant would for sure help you get relief from pain but the reports will be decisive in coming to a conclusion as to why did the surgery fail and the root cause of pain

Let us wait for the reports
Wishing you a healthy life ahead

Do you know what specifically needs to be looked at because I'm still being checked for pain after MVA - can't break that mold even though the surgery failed; The report didn't show the sacral area so we see the doctor tomorrow to ask why certain areas are covered. A blessing I'm so healthy; I have so much weight in abdomen and feel like I have a rock push in right above the tailbone. It's hard to get anyone to understand this all started after surgery. I will report back; you seem to be the only one in my ball park.
By the way, it was after the hernia repair that my breathing changed and compressed lung shows up in film. So many times I reported that wearing a bra or anything over repair restricted my breathing, but, again, MVA is in minds. I fear telling dr what happened when acupuncturer stuck needle in my neck and top of skull. I've never imagined that it would be so difficult to get continuity in care. Thanks for responding. Can you tell me what mesh residue would look like. After bathing, there is a lot of substance at the bottom of tub. This is going to be a good year for you!

Brief Answer:
A clinical examination along with imaging for clinical correlation

Detailed Answer:
Hi

Thanks for writing in again

I would look for the defects in the abdominal muscle and weakness in pelvic floor to come to a conclusive diagnosis.

Nerves are usually not seen on imaging it is more of a clinical diagnosis. so if the sacral area is missed it would not make a major difference.

Chest physiotherapy after a major general anesthesia is always helpful resolving respiratory problems.

If the recent chest xray shows the same changes in lungs you need to see a pulmonologist

Mesh residue will not come out while bathing, the substance that comes out in the bottom of tub would be mucus and secretions of the prolapsed viscera

THanks for all the good wishes
Regards
Dr Kukreja

I have the report - what is the best way to get to you?
You are amazing.
My husband and I feel we've gotten good and bad from Military Hospital - they are overworked; so much time in between appointments and, quite frankly, my case is compliated, so we are searching for a multidisciplinary site.
SI injection did nothing; Something with injections - the very first time I had a lumbar injection, I had fecal incontinence; after the others, also, and then after surgery I'd go non-stop without knowledge then fine. However, when the prolapse returned it comes and goes. One dr thought Cuada Equina -
Please let me know how to sent report and, if you have any connections/knowledge of where I might go where Chronic Pain won't close doors. I'm grateful that you will take the time and share your knowledge. Blessings to you

Doctor Kukreja, please see attached reports and let us know your assessment. Thank you.

My husband sent SPECT however the report was incomplete because the flow was incomplete.
May I ask you who "quarterbacks" in your country? My providers are suppose to but, drugs were their answer. Four years and my once strong body wreaks in pain. I didn't mention to you tgat before MVA I had anterior vaginal walk repair - tge same wall is shared w/colorectal surgery area. Wonderful pain management dr but we were unfair to him hoping he could put together all the pieces. That is not his place and I can't expect not take advantage of him. He's so kind and tries very hard but yesterday he said to take care of abdominal mesh issues and failed surgeries. It must be hard to understand that my husband and I did all the trials to get better because we had no guidance.
You may be interested to know that I went over my prescriptions and saw that I was charged for Fentanol patch prescribed by a dr I wouldn't take an aspirin from. Fortunately the drugstore has a film and signature. I had mailed prescription to Dr w/a letter telling him I know he's younc; hoping to help him as he was more worried about his license. I don't know what the world has come to but integrity and the almighty dollar has made people lose their integrity and forget their oath.
Being off opiates is to be a good thing but, the pain is excruciating. I want to avoid bogging you down with questions not related to your very specialized and outstanding knowledge so, if there is a dr on your sits to whom I coukd start from MVA to present and get guidance, I'd appreciate it. We a are through with military. Four years of neglect - not intentional but they are too busy and delay in trying to help is against me.
I wish I coukd do something for you as you have been so kind and patient. You are a hero in my eyes as you have not given up on me. And, I can't give up because I have much to live for. If you coukd see my lovely grandchildren you'd know. Is there someone I can send great remarks to on your behalf?
My husband said he always wanted to go to India-not sure if you are from there, but, we'd sell everything to get comprehensive help. I hope moderators don't get upset because I'm writing too much, but when a doctor follows up with such careful advice, it is remarkable. Worst thing is chronic pain precented two surgeries from being addressed except w/meds. I was so very healthy. And I know you understand pain. I'm not as strong
as you and the military drs say "no pain, no gain" however, many fine military drs; I was lost in the cracks. Apologize for being personal but, to moderators, please know that you, Dr K, have given me hope and finding you on site was a gift. They are fortunate to have you. Report sent.

Brief Answer:
SPECT Inconclusive

Detailed Answer:
Hi

Thanks for all the kind words and compliments I am honored

I really appreciate your will and the strength to fight the disease

The spect report appears inconclusive

SacroIllietis is one situation which does cause pain in the sacral region But my concern anyway would be to take care of the prolapse and the hernia

There is mention of a CT Scan dated 29Dec2014, Can you share the report of the same ?

It is OK with me you can get back to me anytime

The pain consultant in Illinois had a very unfortunate Cavernous sinus thrombosis and because his right side of the body was affected he was denied malpractice insurance and he has given up practice, yet I will ask him if yoy can contact him for guidance if not treatment

Yes I am from XXXXXXX in case you happen to travel to XXXXXXX just drop in a line and I will arrange to see you personally

We live in a joint family and I can understand the bonding with grand children, please be assured you would be back with good times to enjoy with them

Please forward the CT abdomen and pelvis report and let us see if we could be of some help

With Personal regards
Dr Kukreja

Did you receive the SPECT? The injection was a hunch and did not work. The Pain Management doctor is trying so hard but he is at a disadvantage because he's so kind and wanting to help, but he doesn't have the history. He did say to take care of colorectal/abdominal mesh. We took advantage of him because he's the first doctor to listen, but, too far gone to rely on him. Just need to know if you saw the SPECT and why you say it is inconclusive.
Am on a roll - going to go to Mayo or XXXXXXX for diagnostics. Unfortunate that Chronic Pain label has closed doors but I will not give up because I was treated only for Chronic Pain with opiates. Basically, why is SPECT inconclusive - because of lack of flow?

Brief Answer:
As there is no mention of the prolapse

Detailed Answer:
Hi

Yes I did receive the spect
I too would recommend treating the cause rather than controlling the pain

Spect is inconclusive as we are not able to predict the cause of reoccurance of prolapse, for me it is very vital

Mayo or XXXXXXX should help you

Chronic pain though a vague term, initiates the process of differential diagnosis in the minds of the treating doctors

Do keep me updated on the progress

I believe you are saying the area of the surgery/resection was not visible. Am I right? I also want you to know that the scans are read by residents; in past have missed things, but do not mean to be critical. The SPECT didn't show area of surgery, am I correct?

Sorry to bother you. We are getting organized for Mayo - the SPECT radiologist said the scan was complete but, complete if the doctor didn't request certain areas? And, lack of flow? Wouldn't complete flow be necessary to be adequate?
The side of rectum, where colon was placed, is one of areas of extreme pain; would that or did that show? I'm beating this to death because I want my ducks in a row when I go to Mayo. And, Pain Management doctor would request certain areas be tested that are in his realm; not a colorectal surgeon's. I so believe that the i's and t's aren't dotted and crossed. And you are most helpful. I get different reads on tests; from different specialists. It is unfortunate that I am the Provider, detective, and, you are patient to let me question you because time is very limited once in a doctors office.

Brief Answer:
Probably Yes

Detailed Answer:
Hi

Any investigation for what so ever reason if it does not serve the purpose of the same is inconclusive

In my set up I would have gone for a simple high resolution ultrasound or a contrast based GI Imaging

Please do go for a good clinical evaluation at Mayo and get back to me I will put in my best to help you out

Thank you and apologize as I'm told I'm asking too many questions. I hope you understand I got lost with Chronic Pain label; effort here has been addressed, for 2 years, as chronic pain and I couldn't get that out of doctor's head even with the surgery. You've been so very helpfpul and, until I have conclusive reports, I won't bother you any more.
I did tell customer service that I was sorry, but, that you were phenomenal in follow through. Hope I caused you no harm. Until conclusive tests, my best to you and I have such gratitude for your advise and concern.
