Posted on Fri, 30 Jan 2015
Question: Hello Doctor. I had sexual intercourse with a woman 8 days ago and the condom broke. I pulled out my penis right away. Since yesterday I been having pain in penis after urinating and also dribbling after urinating. I have been pressing hard and very often on my penis to check if there is any drips. Nothing. Should I consider having caught any std?

Hello Dr. XXXXXXX

There is no redness over the glans or tip of the penis . No eruption around it either. The genital area doesn't appear to have any abnormalities. There is no colorful discharge from penis apart from the clear urine like discharge after urination. The pain after passing urine is quite mild, more like a stinging pain.

I am 31 years old. I do have a history of genital problems. 7 years ago I was diagnosed with chlamydia. Did the antibiotic treatment, got tested again and everything came out negative. Since then, I always feel discomfort in my genital area. About 3 years ago I had anal sex with my ex-girlfriend and noticed discharge from penis a week after the anal intercourse. Doctor diagnosed me with some sort of bacteria, was given antibiotics though I never took them. About 2 years ago to this day I've been having problems of very frequent urination even wakes me up at night 3-4 times, hard to pass urine and always the same discomfort in the genital area (penis always moist, and hurts when someone touches it) . 6 months ago a Cystoscopy was done and apparently everything is normal. Did blood tests and everything seems ok.

After the rupture of the condom 8 days ago, I have doubts of having caught Chlamydia or gonorrhea again. From all the problems described I haven't had burning sensation after urination or this much dribbling after urination. Don't know if I'm worrying for nothing. The sex I had 8 days ago was with a sex worker.

Thank you Doc for the fast replies

Brief Answer:
No possibility of any sexually transmitted disease from recent episode.

Detailed Answer:
Hello young fellow,

Welcome again to the forum and thanks a lot for your swift follow up with important details.

I have gone through your complete query and the further important information with diligence. I can read out the complete sequence of the problem and can very well make out that you are quite stressed regarding possibility of sexually transmitted disease.

So to guide you best let me first reassure you that the present problem of the pain in urethra after urination is not related with the 8 days back episode of sexual intercourse. Therefore there is no possibility of any major or minor sexually transmitted disease or infection from that episode.

To tell you more elaborately: your recent incidence of sexual intercourse was protected and careful with your full concern taken to withdraw the penis immediately after bursting. In these type of incidence it is practically not possible to have any type of sexually transmitted disease.

Even to reassure you more: your present symptoms are not related to any of the sexually transmitted disease which can manifest in this way so early. Most of the sexually transmitted diseases which manifest within 7 days of the episode have either discharge or redness or atleast some eruptions. In your case you can notice that there is no such kind of things been present.

Rather after analyzing the complete situation I can make out the possibility of basically urethritis due to nonspecific cause. The symptoms enumerated can be because of the Urinary tract infection (UTI) since last 7 years and then recurring after anal sex with your girl friend. This can be because of both Sexually Transmitted Infection as well simple causes.

In yourself it appears to be due to simple microorganisms present on the skin or due to nonspecific cause. To differentiate the simple cause to that of Sexually Transmitted Infection - there is profuse pus discharge with ulceration or lymph node enlargement in UTI caused by major Sexually Transmitted Infections.

This UTI part can be nonspecific also due to less drinking of the water other than infectious cause.

I suggest you to follow the regimen below to reach to a conclusive diagnosis:

- A proper base line ultrasonography is a must in this situation. An edema in the prostate can be easily found out by the sonograpphy. If the sonongraphy reveals the prostate higher than 30gm then it indicates some infection or inflammation. This can tell us the reason for dripping of urine as well frequency of urination.

- The next step is to go for culture and sensitivity of the urine and semen both. A mid sample (meaning middle of the urination or secretion) is best sample. A proper of test will delineate the exact cause and the sensitive antibiotics so that it can be followed properly for adequate period of time.

- There is another way out that an empirical course of combination therapy of antibiotic is best in this situation. For this tablet Levofloxacin 500 mg daily and Nitrofurantoin 100mg can be taken for a period of 21 days under medical supervision. A repeat test can be done after 1 month. Nitrofurantoin can be extended for another one month of period as maintenance therapy.

- Tablet alfuzosin 10 mg can be added up once daily to relieve the symptoms of pain, irritation and swelling and other of the symptoms which you are feeling during or after urination or in the groin area and prostatic area.

I suggest you to meet a urologist and can discuss all these points enumerated above.

So do not worry about any type of STDs including any major or minor one after the recent episode. Your condition is quite simple and a step by step proper well directed approach will give you best relief.

I hope the above information will relax you and help you in your present problem too. I will be glad to help or if not then can close the query and rate the answer.
