Question: Hi doctor, I have been having hemorrhoids from past two to three years, I might have had three to four times severe pain, I used to take pain killer (tylenol) and it would go in 3-4 days. I also workout atleast 3 days a week. I am not over weight my B

Thanks for your query and an elucidate history.

The hemorrhoids are always painless unless they are thrombosed, and once thrombosed they would cause pain again.

Hence the most probable reason for the pain is anal fissure.

I would advise you to have a clinical evaluation, actual physical examination including the per-rectal and proctoscopic examination to confirm the exact diagnosis.Colonoscopy is routinely done in all such cases as a norm.

The General outline for the treatment is as follows:
-Mild laxatives in a dose that suits your needs.
-Sitz bath in a tub with warm water.
-Treatment of the cause found as per the advise of your General Surgeon or Gastroenterologist.
-High fiber diet.
-Avoid all the foods and the beverages which you must have noticed by this time that either start the problem or increase it.
-Stress and anxiety if present can have effect on the disease and its outcome.
