Suggest Remedy For Nausea, Pain In Arm And High BP

Question: My soon to be 79yr old mother who is 5'2, 200lbs has been feeling light-headed, nauseous, suffering cold sweats, and her left arm feels weak. She has had congestive heart failure twice in the last 2 yrs and polymyalgia the last 5yrs that is currently in remission. (she is off prednisone) She is being treated for high blood pressure. 2 yrs ago she had a para-esophagial hernia repaired with a resection of her stomach which is still partially in her chest cavity due to the length of her esophagus.My question is should I be worried about a heart attack? Her oxygen is 96, bp 122/76 and pulse of 66. Her left arm feels weak, she cannot lift or grasp items. She has no pain or pressure anywhere. She has shortness of breath but that has been ongoing due to her activity level, weight, and arthritis and is no worse since these symptoms started last night. I'd appreciate your opinion!

I have gone through your history. The sort of symptoms you are describing may not be directly related to the heart attack as she is not having any pain/sweating/ uneasiness and her vitals are normal. Considering her age it can very well fit in with shoulder periarthritis or a tendon inflammation like tennis elbow which is not allowing her to raise the arms or grasp things. If that is normal, we just need to check for her nerve entrapment for it.

Considering her past history of heart failure first thing I would do is to rule out any cardiac problem. Although from the history it doesn't look like but just to be sure we should rule it out. At least the baseline with ECG, 2d echo and cardiac enzymes if they are normal at least we are sure it is nothing serious and the rest can be dealt with.

Just get it evaluated once. If not cardiac it is probably musculoskeltal.
