Question: Hello Dr.,
my son aged 18 yrs., having get irritated at the slightest of any incident. throwing objects at sometimes, walking in side the house (room to room ), thinking always something, having less concentration in studies,afraid of being alone in the house, etc.

1. Adolescent behavioral problems - Usually when children real adolescent period, they have developmental conflicts such as role, gender, society, self identification, personality etc. They feel they are grown up and mature, but in actuality they are not. Such a mindset makes them to get irritated at advises and interferences from family members and relatives. Such irritation could trigger irrational behavior. This adolescent behavior pattern should ease and disappear when they mature.

2. Disappointment related issues - People at this age group realize that life is a complicated one. They are actually at the cross roads. They experience so many incidents which are not as per they desire. Education, gender issues, affairs, friends issues, peer pressure etc can disappoint them. Disappointment induces symptoms like depression. Irritability, anger, outbursts, self talking, wandering, loss of sleep, loss of appetite etc are common. Such disappointment results should ease after a week or so if they keep themselves engaged. A good behavioral counseling could do the trick.

3. Attention Deficit Hyperactivity Disorder (ADHD) - is a psychiatric disorder in which there are significant problems of attention, hyperactivity, or acting impulsively that are not appropriate for a person's age. Inattention, hyperactivity (restlessness in adults), disruptive behavior, and impulsivity are common in ADHD. Academic difficulties are frequent as are problems with relationships. You need to give more details about your sons behavior so as to conclude this aspect.

4. Emotional and Behavioral Disorder - Notable emotional and behavior problem while interacting with same age group and society which is uncommon to the specified age group.

5. Schizophrenia / Psychoses

The medicines which you have mentioned are usually prescribed for schizophrenia and delusional psychosis. Please let me know whether he was diagnosed by a psychiatrist as having schizophrenia.

Please go through the above details thoroughly and come up with a detailed follow up regarding your sons behavior.

Hello Dr,

Thanks for the reply.

As requested,
We are consulting psychiatrist periodically.
The following problems were reported to Dr. also.
First some medicines prescribed for him does not work properly or aggravated the symptoms. ( especially day medicines viz. Arip MT5).
He is sitting for +2 exams in the march.

The night medicines put him in sleep for longer duration (wakes up 9-10 am ) and even after drowsiness prolongs. so he is not able to concentrate in studies. few medicines like sizodan works well but sleepiness problem persists.

Our main concern is which medicine/s suits him best which makes him to normal sleep and concentrate in studies (maths) and also reduce his temperament by cordial movements with others.
He always talks about impractical ambitions like joining in IIT, Higher edn. at Abroad etc. ( He was very a high calibre and intellectual student in his early stages) He finds difficulty in Maths.

Other habits : less hygenic, no regular haircut and bathing. fond of eating (abnormal) and gains weight aroung 15-20 kgs over a period of 6-9 months and
hunger/sleep reduces when we suspend the medicine but significant behavioural change ( slightly violent when advised )

I can understand your circumstances especially when your son is preparing for +2 exams. In such a scenario sleepiness, lack of interest and impulsivity are not favorable.

He is highly ambitious but his current scenario is irritating. He is unable to do his studies because of the medicines and he is dejected. Dejected people always are irritated and dislike advises. He needs answers to his questions.

There are no better medicines. Psychiatrists prescribe medicines after thorough investigation and possibly trial and error methods. Schizophrenia and anti-psychoses medicines are known to inhibit drowsiness.

In my experience, along with medicines, we always make them understand the situation. We advise them, to overcome the drowsiness, motivate them to play agile games like basket ball, badminton, hockey etc, eat a balancing diet and interact wisely with friends. When he is able to do these things sincerely, he will have more appetite, resultant - less drowsiness and clarity in thinking. Yoga also helps.

When they know that they have this problem and they should come out of the situation without the help of others (when they are cornered, as they are young), they automatically become agile and active. A few days of positive attitude and practice automatically provides a new habit and further change in brain dynamics.

The above method is the best known. If this method continues, the medicines can be tapered down and stopped.

Please appraise him about the situation and motivate him so that he become more active. You can consult a good life style counselor for motivation if you cannot motivate him enough.

I am sure this will give you good result.

Dear Dr.,

We have put my son to counselling 2 -3 times where he seems to be normal and slightly shows some kind of resistance and claims normal.

He came for the initial counselling with the Psychiatrist and counselled for and obliged to take medicines. after some time due to its adverse effect as stated earlier he refused to take. Based on his Dr's advise we give medicines at supper time without his knowledge till now. This is the current situation. during exam times only we suspend medicine/s.

We are residing at trichy,tamilnadu. Is it possible to discuss /counsel with you personally or over internet ( skype etc.) . The boy is willing to take counsel with a new Dr.

I can understand the problem. It is natural for the patient to resist treatment claiming they have no psychiatric problem, because we see psychiatric problem as a social and individual health disease "with no return". But in fact, there is cure.

This is what is to be conveyed to your son by experts in expert manner. Psychiatrist must have done so.

Your son should be brought into a behavior pattern where he himself willingly take the medicine and do the needful mental and physical exercises (CBT and VT, as prescribed by me).
