Suggest Remedy For Infection After Plastic Surgery

Question: Dear Doctor,
I have a problem I had plastic surgery to remove a dermatafibroma on my left anterior forearm.
I have sutures in it. Well, I am a nurse and went to work this weekend 9 days after surgery. I bought sterile supplies from CVS everything. Now I am seeing my surgeon on Thursday at 12noon. I think after all my diligent covering with steristrips,tegaderm,kling, ace wrap I may have a small infection. I just took
Zithromax I had here at home I am a nervous wreck . I just keep thinking I can get sepsis maybe far reaching but I had to do something My incision is 1 inch long 1cm at the end has edema on both sides
I know nurses make terrible patients I very OCD personality What do you think? The surgeon will not be in to Thursday Thank you, XXXX
Everything went good until I wen to work I was extremely careful I thought.

I am Dr. Kakkar. I have gone through your query and I have understood it.

If I was the treating doctor I would have also put you on empirical antibiotic therapy either right after surgery or at the first sign of infection (redness, swelling, warmth, pain or tenderness). You did the right thing by starting empirical oral antibiotics. If you were my patient I would have advised you to continue it for 5 days.

You may use Povidone Iodine solution for wound asepsis, before covering it with sterile dressings.

Keep a watch on the signs of inflammation if there is any increase in redness, pain, swelling etc. The oral antibiotic would likely take care of the secondary bacterial infection. However, if the redness and pain continue to progress, it would be better to remove the stitches and let open the wound, to drain any pus/infection along with a full course of an oral antibiotic. If at all it is required to remove the stitches, the wound would then be allowed to heal by secondary intention.
