Suggest Remedy For High Fever And Flu

default
Posted on Fri, 30 Jan 2015
Question: Hi Dr. Im a 34 year old male. Not over weight. Last march, I felt like I caught a very strong flu. I did not throw up or get an extremely high fever, but felt quite ill. Even confused at times. I took 1 week off. During that time I noticed my eyes were slightly cream colored or yellowish. Also very glossy. They didnt look well at all. During that time we found an elevated billirubin and slight ferratin elevation. all other liver enzymes were in the normal range. an abdominal ultrasound came back normal. hep a-c. negative. that was 9 months ago now and my eyes are sometimes still yellowish. apart from that my energy levels have not fully went back to normal I feel. sometimes I also have mild discomfort in my upper right area. cant quite pin-point the area. ferratin went back to normal range soon after the episode. Also all stool test were ok. Full blood work ok as well. Have to wait 1 year to see a gastro. Little worried. I was told its probably Gilberts but I should feel fine if Gilberts. Also, I dont think Gilberts would start the way this all started. Im a community health care worker- exposed to alot of illness and germs. Perhaps this is where it all started.

Detailed Answer:
Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.

It looks like XXXXXXX Syndrome only. But I would like to have some more information before I can confirm it.

1. What are the direct and indirect bilirubin levels?
2. What is the reticulocyte count?
3. What is the ceruloplasmin level?

Kindly get back to me with these information.
Regards,
Dr.R.K.

Brief Answer:
You are having XXXXXXX Syndrome only

Detailed Answer:
Hi,
Thanks for uploading the reports.
It looks like you are having XXXXXXX Syndrome only.
It is a condition in which the indirect component of bilirubin will be high and the liver enzymes will be normal.
In your case only the indirect component of bilirubin is high and the liver enzymes are normal which points towards XXXXXXX Syndrome.
Hemolysis (breakdown of red blood cells) can cause similar picture. That's why I asked the reticulocyte count to see whether you have hemolysis. But I could not find reticulocyte count in the report. But it has to be normal only as your hemoglobin is normal and the LDH is not high ( In hemolysis, hemoglobin will decrease and LDH will increase). So hemolysis as cause of increased bilirubin is ruled out in your case. XXXXXXX disease can present with hemolysis. To rule this out I asked for your ceruloplasmin report. It is normal in your case and also for the reason described above hemolysis due to XXXXXXX disease is also ruled out.

In XXXXXXX syndrome, the bilirubin level fluctuates. Some of the causes of an increase in the bilirubin in XXXXXXX are dehydration, starvation and stress. Any one of these factors is responsible for the fluctuation in your biirubin levels.
In XXXXXXX syndrome total bilirubin levels never exceeds 5 mg (about 80 umol/L
). In your case it never exceeded 45 umol/L.

Ferritin is an acute phase reactant which increases when there is infection/ inflammation. That is the reason why it increased when you had flu.
XXXXXXX syndrome is a benign condition and does not require any treatment.
Most of the serious conditions already ruled out ( hep B, XXXXXXX disease). Your Hep C and hemochromatosis report still awaited.
Your albumin globulin ratio is more than one and your ultrasound scan is normal which indicates you do not have any chronic liver disease.
So there is no need to worry.

I hope that answers your question.
Let me know if I can assist you further.
Regards,
Dr.R.K.

Thank-You for taking the time to read my reports and assure me that I should not worry soo much. I was wondering if I should be re-tested for hep A or E antibody to be sure.

Stress has been high since last march because I notice my eyes have not been white since then. If they go back to white, I would feel so much relief. Of course people notice my eyes.

Many Thanks.

Brief Answer:
No need to retest Hep A and E

Detailed Answer:
Hi,
There is no need to retest Hep A and E. These do not last for such a long period.
Lets wait for Hep C and hemochromatosis report.

Your bilirubin level is not very high to cause yellow eyes. You might be probably having a muddy conjunctiva.

Let me know if I can assist you further.

Dr.R.K.
