Brief Answer:
1/2 tablet

Detailed Answer:
Hello. Thank you for your question and welcome to HCM. I understand your concern.

If this is the first time that this phenomenon is happening, I would recommend you not to worry, because blood pressure can "jump" even in physiological situations, such as physical activity, psychological stress, or after having excessive amounts of caffeine-containing beverages. If this is not the case, then take the other half of your blood pressure-lowering tablet, and put it under your tongue, and keep it until it is totally melted. This is an effective measure for a drug to act faster because it goes directly to the bloodstream, without being metabolized first in the liver. If even with this measure, after waiting 20 minutes, you get high blood pressure figures such as those provided, then I recommend you to go to an emergency room nearby, and IV nitrates or sodium nitroprusside will be administered, to effectively lower your blood pressure. I would also recommend you to discuss this episode with your cardiologist in your next meeting, so that the blood pressure-lowering medication should be modified, as a strategy, add/change drugs, in a manner that these episode do not happen again, and blood pressure is kept under 140/90 mmHg.

I hope this is of help. If you have follow-up questions, I am happy to help. Please rate the answer, if you do not.

Kind regards,
Dr. Meriton
