Brief Answer:
Gallstones.

Detailed Answer:
Hello!

Thank you for the query.

Upper right abdominal pain which radiates to the back is very characteristic for gallstones. You may have also nausea and lack of appetite with this pain. In most cases such pain is triggered by fatty foods or alcohol intake.

It does not seem to be associated with recent appendectomy.

In some cases such pain can be also a symptom of acute pancreatitis. However the pain is really strong, vomiting is almost always present and abdomen gets really hard.

I suggest you to consult your doctor. Abdominal ultrasound (not CT), blood work, liver test, bilirubin level, amylase, urine analysis, urine amylase should be done.
In case of stones detection, surgery (not urgent) is necessary. In most cases it can be done laparosopically.

Hope this will help. Feel free to ask further questions.
Regards.

Thanks! Should I inquire with my GI specialist who performed the appendectomy or talk to a GP?
