Suggest Remedy For Constipation And Abdominal Bloating

Question: I have CONSTIPATION, with bloating, for at least three days, very little output. Very, very tight analsphincter, was taking magnesium supplements but stopped with all other supplements because of recent diagnosis of gout. Concerned abbout continued CONSTIPATION no output. Taking phazyme hastime for gas and ranitidine, prescribed by doctor.

I appreciate your concern

PPIs like pantoprazole, rabeprazole are much better than H 2 blockers like ranitidine with respect to GERD. If there is no abdominal pain , tenderness and fever then I would recommend a two week empiric course of a PPI like Esomeprazole and a gastro kinetic kinetic like domeperidone or metoclopramide to see for improvement. Sodium picosulfate and Ispaghula husk combined with cremaffin syrup should also be added to the regimen. Most likely you will have a bowel movement with this medication within 4 to 6 hours. In severe constipation a kleen enema Per rectal can be done.

If there is absolute constipation , abdominal cramps, rigidity and tenderness associated with fever then x ray erect abdomen, blood counts and basic metabolic panel including electrolytes can be ordered in consult with your doctor. Local examination including a DRE (Digital rectal exam) can also be done. A surgical opinion should be sought to look for intestinal obstruction.
