Pyelonephritis usually causes lumbar pain whereas constipation is usually pain around the umbilicus or in the right lower left/ right abdomen.

Pain because of pyelonephritis tends to remain up to 6 weeks but in severe cases my patients have had it beyond that as well.
Red flags in pyelonephritis are the following

1. vomiting and high grade fever
2. loose motion
3. Fall in blood pressure
4. pain in the opposite side indicating spread to the other kidney

For relief from constipation I recommend that you eat food rich in fiber like vegetables and fruits and also take lactulose 20 ml at night after dinner

Please do not take ibuprofen it is toxic to the kidneys. I recommend acetaminophen or tab tramadol for the pain but tramadol will require a prescription.

Cranberry juice is only useful in cystitis and not pyelo

There is no need to restart antibiotics if you have received 14 days of antibiotics. You might want to repeat a urine routine to check if you are cured. Presence of pus cells means that it will require further treatment with antibiotics
