Posted on Thu, 23 Jun 2016
Question: my husband has recently been put on Blood pressure medicine and now he is having these surges that wake him up and keep him from sleeping at night. Every night, he just got over a very bad case of vertigo, but that is gone and these surges won't stop. What could be the problem? He has had a cat scan and everything was fine.

Explained

Detailed Answer:
Hello and Welcome

I appreciate your concern

Blood pressure medication usually require 6 weeks for optimal control when used regularly. Though the antihypertensive effect starts on day one but adequate normalization requires at least 6 weeks. If the BP is persistently high with evidence of end organ damage or a TIA then hospital admission should be considered. Hydralazine, labetalol or nitroglycerine are in hospital medication used to control very high BP. For the vertigo , further assessment is needed to determine whether its due to a central or peripheral cause and managed accordingly. The surges need to be monitored for by doing home blood pressure monitoring e.g 3 readings a day and assessing the titration of BP meds dose accordingly.
