Brief Answer:
Further investigative work up is required.

Detailed Answer:

Easy fatigue ability and weight loss of 4 months is a serious issue and should be carefully evaluated.
Vitamins and water alone doesn't help.
It's advisable to consider following set of tests to evaluate further cause of action.

I generally prescribe these investigations to my patients with such symptoms.

1. Complete blood count
2. Thyroid profile
3. Ultrasound abdomen
4. Fasting and postprandial blood sugar
5. Complete Urinary examination
6. Liver function tests
7. Renal function tests

Further management if any is largely based on test results.
Review with reports to determine further course of action.

Hope I have answered your query.
Let me know if I can assist you further.
