Question: Hi doctor, When I have been to foreign land , in attempt to loose virginity, I had a sex with a hooker. Before involving into sex, she has given me some kind of cream to apply for penis. So I did. I was drunk didn't remember much but when I had condom to penis, it was broken after 1st penetartion later after 2 min realised that it was broken and again used a new condom. Exactly After 4 weeks, I have a got a severe headache which can never be explainable and visited local GP in XXXXXXX told me that it was a sinus but subsided after 3 days. After 54 days of exposure, I took test for HIV in ICTC center in XXXXXXX got negative. After that, I have not tested. But after a year, started getting fever, mouth ulcers and swollen lymph nodes under armpit frequently and this was increasing anxiety in me, Due to this I am not able to sleep and concentrate on work in office. My question is how reliable tridot test after 54 days exposure in ICTC center. Are they using 4th gen test? Are my symptoms are common symptoms or related to HIV? That is my first and last sex ever in my life.

Fever, mouth ulcer and lymph node enlargement can occur to anybody in the absence of HIV. Not to worry.
In ICTC, 4th generation test is not being used so far.
Your rapid test done at 54 days after the exposure can not be considered as conclusive. Window period can be extended up to 90 days. Please repeat the test once again to rule out HIV.

In ICTC they are using 2nd generation HIV tests

In ICTC they are using 1st and 2nd generation HIV tests. They are also highly reliable up to the extent of 97% and above. When two different test came nonreactive then the reliability comes to the extent of nearly 100%.

Sorry for asking many questions.

Actually, in ICTC, they took blood from intravenous. What kind of tests and how many tests do they do for HIV, in order to assure the person is negative. As per CDC, the results are conclusive on 8 week test irrespective of generation. Is that correct?

I just want to know that have you ever come across the person who tested for negative on 8th week and then turned out positive after 3 months in your practice?

In ICTC, even though, there are three different types of HIV test kits are available for each patient, They will not go for the second and third test if the result had come nonreactive. Only when it came reactive they will go for further tests in order to reconfirm.
If it came nonreactive they would advise to repeat the test, if it falls within the widow period of the test.
HIV tests 8 weeks after the exposure is reliable, but can not be taken as conclusive. It can be taken as conclusive after 12 weeks, as few individuals would have a delayed sero-conversion.

I had come across with very few incidence where an individual became positive by the end of 12 weeks who were negative earlier.
