Question: Hi I'm 8months pregnant..i already have fissure problem. Now from past one week the pain in intolerable and there is too much swelling and pain ... What to do .. I can't give pressure as well.

Hi.
Thanks for your query, albeit short and specific.
To recapitulate: 8 months pregnancy - intolerable pain in fissure in ano for past 1 week - so much swelling too - can not give pressure - wants to know what to do.

I would suggest you the following:
- Sitz bath in a tub with warm water.
- Clean the area thoroughly after passing motion with plain water so as to wash off the traces of feces which remain and cause more infection and inflammation.
- Apply Lignocaine or Xylocaine anesthetic jelly just before passing motion to get less pain and strain.
- Since the pregnancy is 8 months safe antibiotic can be taken as well as paracetamol or Ibuprofen can be used to reduce the pain and swelling.
- Clinical examination by a General Surgeon to decide and confirm whether this is fissure-in-ano or prolapsed and thrombosed piles.
- Mild laxative only if there is constipation.
- Normal full diet as before and avoid the foods and beverages that cause constipation or increase the pain.
- Further treatment after examination and confirmation of the diagnosis as required.
