Brief Answer:
Management of acne

Detailed Answer:
I have reviewed your photographs and you are having pimples as well as scars.
Isotretinoin is a very effective medicine for severe acne. You have mentioned about its use. In what strength and for how long have you used it?
If I was your treating doctor I would recommend you to take tablet minocycline 100 mg once daily along with application of gel containing combination of adapelene and benzoyl peroxide in minute amounts at night over the scars as well as the pimples.
To start with apply the gel for one hour and gradually increase the duration by 1 hour till the time over night application becomes comfortable.
These medicines are prescription drugs and you need to see the doctor in person to get them.
Also use ketoconazole shampoo twice weekly if you are having dandruff.
For the scars you can use the above mentioned combination but you need to go for either dermaroller treatment or ablative laser treatment for better results.
Dermaroller can be used once your acne becomes inactive.
Moreover keep your face dry, avoid oily creams, avoid oiling of scalp, use cosmetics labelled as noncomedogenic and reduce the intake of sugary foods.
I hope to have answered your query and would be happy to clarify any further doubts.
Regards
Dr Asmeet

Brief Answer:
Tablet Minocycline and benzoyl peroxide adapelene combination gel

Detailed Answer:
You can start with tablet minocycline 100 mg once daily after getting your baseline liver function tests and Antinuclear antibody test done.
Also apply gel containing combination of adapelene and benzoyl peroxide at night on the pimples but in very small amounts.
Many brands of the same combination are available in the market like minoz BPO gel or epiduo gel. You can go for any of the above mentioned gel.

Brief Answer:
Face care facewash, cetaphil moisturising lotion & faceguard sunscreen

Detailed Answer:
You can use facecare face wash twice daily, cetaphil moisturising lotion as moisturiser and faceguard sunscreen gel during the day time.
Use epiduo gel on the pimples for half an hour at night in the beginning, continue this for 1 week and once you are comfortable with it increase the time to 1 hour and gradually to 2 hours. Further apply it for 2 hours only at night for 1 month.
