Suggest Remedies For Hematoma In Lower Back

Question: My husband and I were in an off road vehicle accident almost 3 years ago. He took a blow to his lower back. He suffered a VERY severe hematoma. It ranged from his middle back to his knees and wrapped around his torso. At least 50% of his body was purple. Severe swelling occurred. He's been in pain ever since, and life has not been the same for us. He's been tossed back and forth between several doctors for almost three years (surgeons, pain management, family practice, physical therapy, etc.) ...even going as far as having a surgery for herniated discs last year. Recently, he had a soft tissue MRI that confirmed that he, in fact, still has a hematoma in his lower back. His back swells up and down on a regular basis. I know this is serious....but HOW serious is it that he is still having a swelling problem? It's about a softball size depending on the day and his daily activity. He is 26. And what kind of doctor should he see that can help him get this RESOLVED? Can you drain a hematoma after this long? Doctors he's seeing now are a bit perplexed about his situation and appear to be reluctant to offer sufficient treatment. (also, side note, he fractured his spine in a car accident 10 years ago--an injury that he had no lasting effects from).

Hello,
I have studied your case. Hematoma lasting for such a long time is quite unusual. It can still be drained. You should see a orthopaedic surgeon. I think this may be a new hematoma and could be due to fresh injury. If there is only hematoma then it is not a serious condition. I have following suggestions.
1. Get his clotting factors and coagulation profile. So that we can find out why hematoma is not absorbed.
2.Hematoma aspiration once cause is established.
3.physiotherapy for back muscle strengthening exercises.
4.Hot water fomentation

Thank you for your advice. What do you mean by a "new hematoma and due to fresh injury"? I would say it's definitely not "new" being that it's been there for 3 years. Maybe I'm misunderstanding. Also, the process for # 1 would be get blood work done, yes? Let me know. Thanks.

Hello,
It is very very rare for hematoma to be there for three years. As you said that he has gone through surgery of the back also so hematoma would have drained at that time. If there is abnormal coagulation factors then it can cause development of spontaneous hematoma or it can prolong healing of previous one.
