Suggest Remedies For Acne Vulgaris
Question: Suffered from acne vulgaris (as mentioned when I visited doctor)but the acne comes and goes.first I used to use fluctivate cream only when on attack of severe acne days..the cream cured the acne in two days completely..after two days i used to stop the cream then again the itching and burning and rashes on face would appear..I again had to use the cream..iv never had bad pimples..but rashes and itching and irritation occurs every now and then with rough skin which happen to be v small pimples mayb..now after almost 6 7 years I am totally dependent on this cream since nothing seems to cure and this rashes occurs every now and them.i want to discontinue it but can't since my skin problem doesn't seem to heal for the past 10 years..please suggest me what to do to completely get rid of this.since M fed up of my skin problems and also I want to get rid of my dependancy on this cream.

I am dr.kakkar (dermatologist). I have gone through your query and I have noted your concern.
Flutivate cream (fluticasone propionate 0.05% cream) is a topical steroid which should never be used on face for so long. Long term use of topical steroids on face can lead to side effects like acne, skin thinning, telangectasia, persistent redness and flushed appearance and photosensitivity etc(acne rosacea).
Though flutivate cream is anti-inflammatory i.e reduce redness but it is not an antiacne medication and on repeated use its side effects become more prominent with time and overshadow the initial benefits the cream provided. Therefore, you should certainly discontinue this cream.
I would suggest you to use a gentle cleanser for face wash e.g Cetaphil cleansing lotion, twice daily. Avoid soaps.
You must continue using a broad spectrum sunscreen, specially when outdoors e.g Suncros tint Or suncros soft sunscreen.
For acne you may use clindamycin 1% gel e.g Clindac A gel, day time. At night I would suggest you to use adapalene 0.1% and clindamycin 1% combination gel e.g Acnovate gel Or Deriva -CMS gel.
You may also take an oral antiacne antibiotic e.g Microdox -LBX 100mg, twice daily. It would help control acne faster in combination with topical antiacne medications.

Doc I wanted to add that I am under medication for irregular menstrual cycle.will these medication be of any harm if i am using it both at the same time.
Second ly I was referred adapalene gel 7 years back by doctor which made akin vv dry and red and irritated and it was after that the doctor referred fluctivate cream and M using it since then.
Should I use adapalene gel again? Also plz tel if I can use both oral medication at the same time.

These medications can be used along with your other medications.
Adapalene is first line treatment for all forms of acne. Adapalene is well tolerated in most patients but it can cause skin irritation, redness and skin peeling as a side effect in some individuals. Therefore patients with sensitive skin may start with alternate night applications and later with nightly applications.
The doctor who gave you flutivate cream for skin irritation due to adapalene must have given you for a few days only. The creams was not meant for regular use.
Since you have a history of skin irritation with adapalene therefore you may start with alternate night application of use Deriva CMS gel, going on to daily nightly applications once your skin gets used to it.
You should use Clindac A gel during day time and may also take Microdox - LBX 100mg capsules twice daily along with your other medications.

Doc I wanted to ask regarding application of the gel.Since the acne comes and goes should I apply it during acne outburst or even when there is no outburst
?should I stop using it as soon as the acne stops or should I continue applying it even in good skin days ?

Doc lastly I wanted to ask that for how long should I use the oral medicine microdox lbx 100 mg..? Iv bought all the listed medicines nd have started using and applying it from today.
