Suggest Remedies For A Open Wound On The Foot

Question: I am a healthy 57 year old male, 6 feet, 200 pounds, very active. I have recently been on vacation this past week at the beach in South XXXXXXX While I was there I cut my foot. A few days later my foot started to swell. The swelling is not localized around the cut, but is in the thick part of my foot above the cut and in my ankle and calf. There no pain except when I step where the cut is. The swelling gets much worse when I am upright for longer periods to walk or stand. If I elevate my foot the swelling decreases significantly but does not go away. I had some antibiotics (Suprax 400 mg and some Bactim DS ) and started taking them 36 hours ago. I cannot tell a difference. There is no color change and no heat in my foot. I do not have an elevated temperature. What should I do?

I have read your history and understood your concerns.

My thoughts:
Since there is a cut and swelling, there is surely a problem and hence I would advise you the following in spite of no elevation of temperature, general or local.
- Tests of blood for sugar, CBC, kidney functions.
- High resolution ultrasonography with color doppler for the affected limb. This will help to know which plane the edema is spreading and whether you need further management of say incision and change of antibiotic or so.
- Clinical evaluation by a Doctor, preferably a General Surgeon.

This is so as some of the bacteria can be gram negative and infection with these may not elevate the temperature. Deep seated infection can show normal skin.
It is better to be sure about it.
Till you see a Doctor in ER continue the medicines which you are taking.
