Brief Answer:
The features are suggestive of parkinsons disease

Detailed Answer:
Hello,
I have gone through your question and old prescription.
The symptoms are of parkinsonism.
Gait difficulty, slowing is features of parkinsonism.
Early falls and autonomic features may be due to multiple system atrophy. The drug response in such a case is difficult.
The drugs available are levodopa / carbidopa, amantadine, pramipexole, rasagiline, entacapone.
The dosage and combinations are tried one by one and requires lifelong therapy.
Other options are surgical, which requires detailed examination of this patient.
Hope you found the answer helpful.
