Suggest Medication For Chronic Sinusitis

Question: I have a bad sinus and usually have 2-3 bouts of this throughout the winter months. Normally it takes a couple of rounds of medication and also sometimes a shot or 2 in the DR office. Since all are closed today as it is XXXXXXX 1 I was looking in my medications on hand and have several different left overs. I know that I normally always end up on Prednisone and I cannot remember which of the others to take in combination with this.
Amoxocillin 500 mg
Biaxin 500 mg or 800 mg
Cephalaxin 500 mg
Doxy Cycl Hycl 100 mg
Lo hist 12D 6-45 mg

Can you please advise as to the best combination?

Hi and thanks for the query,

I understand this is already a chronic condition since you have been having this for quit some time.

Antibiotic choices in this case should be directed by cultures of secretions from these sinuses. The specific antibiotic needed to treat the identified bacteria that shall be properly prescribed.

It might be interesting to know if you have already taken a treatment course for 3 to 4 weeks? This important since with chronic sinusitis, taking anti biotics for about a week will not really serve much.

Amoxicillin for a start , 2000mg daily is appropriate added to Prednisolone. It is imperative to see your doctor, get your nasal passages checked for presence of absence of nasal polyps or other condition that could predispose to chronic sinusitis, and getting new X rays of the paranasal sinuses. This is important to exclude complications like pus collected, which if left without drainage, could easily spread the infection to other areas like the brain (meninges).

Amoxicillin is first line as anti biotic choice, to avoid resistance that might arise of use of other antibiotics if not properly identified, and if they are not guided by anti biotic sensitivity results.

I suggest starting with Amoxicillin, seeing your doctor for reevaluation and appropriate x rays, as well as getting specific cultures of sinus collections for germ identification and antibiotic sensitivity patterns
