Question: Dear Dr. XXXXXXX
Hope all is well in beautiful XXXXXXX I am writing to you on behalf of your patient - my father Mr. XXXXXXX
You diagnosed my father with Alzheimer's disease in April last year. We traveled to XXXXXXX again in October for a six month follow up. His condition has been stable overall with no much improvement but notably, thanks to God the deterioration of his condition hasn't accelerate fast for the worse.

You provided him with one year prescription which is about to finish in couple of weeks. We are requesting another prescription from you so we can continue with medication.

We don't see a need to travel to XXXXXXX at this time but if you think we need to do so please let us know.
I sent you the copy of the prescription on your yahoo e-mail account. I couldn't attach it here through health magic.

Best Regard,
XXXX

Following medicines are needed.

Hi,

Thank you for getting back.

I am glad to note that your father is not deteriorating and is keeping well.

As of now, if I were seeing him, the following medicines would have been prescribed:

1. TAB. METHYLCOBALAMIN 500 mcg once daily after breakfast for six months.

2. TAB. DONAMEM 10 mg once daily after breakfast for six months.

3. TAB. ECOSPRIN 150 mg once daily after lunch for six months.

4. TAB. ROSUVAS 10 mg once daily after dinner for six months.

5. TAB. NOOTROPIL (piracetam) 800 mg twice daily after breakfast and dinner for six months.

There is no need to come for follow up now. Review/correspond after six months.

Best wishes,

Thanks for the advice. Kindly can you explain the functions of the each of the five medications you prescribed?

I noticed that ROSUVAS has been removed from the list but it was prescribed last time. Is there a reason you removed it?

Best Regards,
