Question: My father had arterial seclerosis and died at 44 of a heroine overdose. He had a bad lifestyle though. I'm 6ft 175 don't smoke and hardly drink although I had second hand smoke for 18 years. It is a little worse when I lay down but not like choking just like I need to use a little more Stregth but not really gasping of air. I also have a hard time with sexual activity like if I go to masterbate as soon as I start to get an erection I get shakey and fall to the floor. I'm not a very anxious person I took norco two times a day 500 mg from ages 14-18 been clean for two and a half years. So my concerns are can this be an issue with my arteries? If so what blood tests can my doctor run to rule it out. Also I have a vitimin b12 defciency reading of 353. And what is your opinion of my doctors diganosis of peribronchial cuffing being caused by reoccurring childhood asthma? Is this likely or with all the other symptoms it my be somthing else?

Brief Answer:
Arterial disease is extremely unlikely in your case.

Detailed Answer:
Hello, Sir.

I can certainly understand your concerns.

I worked through your query and attached reports in detail.

The possibility of arterial disease is extremely unlikely in your case, considering your age.Atherosclerosis does not occur in the young.

The majority of your symptoms are related to low vitamin B12 and possible allergic asthma.

Yes, indeed peribronchial cuffing is a significant pointer to Bronchial asthma.

Exposure to second hand smoke is a significant risk factor for the development of peribronchial cuffing and asthma.

Anxiety has nothing to do with your symptoms.

Symbicort plus Singular is an excellent agent to help resolve the asthmatic symptoms.Likewise Vit B12 correction is of great help.

With the above 2 medications, your symptoms should gradually improve and you should be fine soon.

Avoid exposure to smoke and house/environmental dust.

Post your further queries if any.
Thank you.

How long does take tipically for the lungs to heal from peribronchial cuffing with treatment? I've been on treatment for four days now, also is there any conditions besides asthma I could have?

Brief Answer:
Asthma is the major cause in your case.

Detailed Answer:
Hello, Sir.

I can certainly understand your concerns.

Peribronchial cuffing is due to thickening of bronchial wall secondary to accumulated mucus in the lower airways. This is seen in Asthma.

It may take 3-5 weeks for the lungs to completely heal from this peribronchial cuffing(radiologically)although symptomatic improvement is evident from 2nd week onwards.

There can be wide variety of causes which lead to peribronchial cuffing, but in your case Asthma is the major cause.

Post your further queries if any.
Thank you.

When I eat anything I seem to deem worse like the chest area feels weaker and breathing is slightly more difficult, is this common in asthmA induced peribronchial cuffing? Also my issue with feeling like fainting as I am starting to develope an erection during masterbation, what can cause that? It is of great concern to me. Thank you for your time

Brief Answer:
Symptoms are due to increased work of breathing.

Detailed Answer:
Hello, XXXXXXX

I can certainly understand your concern.

In asthma, there is increased work of breathing by the intercostal chest muscles, since they have to expel air against dynamic obstruction, caused due to obstruction of airways.

This may cause some difficulty while eating. It may cause some kind of exhaustion.

Fainting like sensation could be partly related to asthma due to minor disturbances in air flow and oxygen supply to demanding organs during erection or masturabation.

Do not worry, all these issues will settle down with time.

Symptomatic improvement occurs with treatment.

Just avoid exposure to irritant smoke and dust.

Complete the course of Vitamin B12 .

Post your further queries if any.
Thank you.
