Detailed Answer:
Dear
Thanks for contacting with your health concern and please furnish following details before I can recommend some result oriented medicine for you:

1. As there is no alternative to hormone in any -pathy so one has to continue with thyroxine and all other herbal approach will help in general well being and alleviation of symptoms, so before advising you regarding dietary regime and medicine for hypothyroid please upload your thyroid report so that I have an idea of your disease state.

2. As the problem is chronic what was the opinion of ENT doctor [chronic rhinosinusitis]? and is it associated with any addiction [smoking]? or with weight loss/reduced appetite, fever?
i. Any associated toothache, headache, eye strain, facial pain and pressure, nasal discharge, poor sense of smell, bad breath?

PS. In nut shell you will require Immunomodulation [Rasayan] treatment for the same

3. Going through your report there are two possible diagnosis mentioned:

i. Acanthosis nigricans: which is more common in people who are overweight?, have dark skin [present] and/or having diabetic or prediabetic condition [check with your blood sugar]

ii. Erythema dischromium perstans: mostly affects dark skin tanned persons and often is related with whipworm infection, any reaction to drug/medicine?, and is resistant to available treatments.

Do revert back with your concerns so that prophylactic aspect of each disease can be suitably imparted and incorporated in your daily routine.

Brief Answer:
further line of treatment and management

Detailed Answer:
Dear
Thanks for reverting back

1.I do suggest to go for another evaluation of Thyroid Function Test from a different laboratory [Lal Path Labs]

i. Since the dose of thyroxine is individualized on the basis of clinical response and laboratory tests thus regular monitoring of T.F.T. [thyroid function test] is necessary to start/change or modify the dose. So cannot comment upon the same
ii. Before switching to any form of alternative medicine, keep in mind the following:

-. There is no substitute of hormone in any –pathy thus check regularly with thyroid values every 3 months

- Take: iodized salt,sea food,garlic,onions,meat,chicken,salmon,tuna,whole unrefined grains,dairy products,banana,egg,low fat milk,mushrooms,guava, carrots,cucumber,black beans,navy beans, red kidney beans,split beans,black pepper,cinnamon,turmeric,mint, coriander,apple, apricots, olive oil,cherries,dates, kiwi,papaya,pineapple,brown rice,prunes,coconut oil

iii. Avoid: linseed/flax oil,peanuts,walnuts,turnip,green mustard,mustard, cauliflower, peaches, coffee,spinach,cabbage,soybean,canola oil,radish,sweet potato,white bread, white flour, junk food, pears,millet
iii.     You can practice; suryanamaskar, yogmudrasana, sarvangasana, matasyasana, bhramri pranayam.

2. Regarding Sinus problem, one can advice the following:

i.Avoid: Heavy, slimy and acidic food, curd, mustard oil, excess physical exertion, sunlight-dust areas, exposure to strong winds, day sleeping, smoking . Potato, green leafy vegetables, cold water/food

ii.Take: Old brown rice, soup of moong dal, black grapes, cow’s or goat milk, garlic, lukewarm water to drink, black pepper, dried ginger, honey, mulethi, launga

iii.Take Raw ginger+ raw turmeric: [2 gm each] and fry in cow’s ghee [6 gm] and thereafter add 6 gm jaggery and take at bed time [during night]

iv. As the pathology is chronic thus take IMMUNOMODULATION (Immunity) i.e. Rasayana drugs (Rejuvenation/Immunomodulators) should be instituted in your management as part of XXXXXXX (palliative) therapy i.e. which corrects the dysfunction of tissues without disturbing the functions of normal tissues, also Rasayana drugs potentiates humoral antibodies, by increasing serum Immunoglobulin’s such as IgA,IgG and IgM.

a. high protein diet ( egg (white), soyamilk, pumpkin, squash, watermelon, peanuts, almonds, if non-vegetarian (meat) are choices , legumes, sprouted pulses/cereals, milk with protein powder , and deep breathing exercises [anulom vilom]
b. Vitamin A: helps to maintain Thymus gland which is a powerful antiviral gland: thus XXXXXXX carrots, cantaloupe , egg yolk, apricots, pumpkin, papaya, are ideal.
c. Vitamin C: is needed for proper functioning of immune system: amla is best
d. Vitamin E: is a powerful anti oxidant which helps to stimulate antibody production: peanuts, dried apricots, cooked spinach, taro root can be taken
e. Vitamin B is important for the activity of phagocytes: eggs, cheese, pistachio, raw garlic, sunflower and sesame oil.

PS.
i. ENT Doctor is in better position for further prognosis, but preventive and prophylactic treatment and Management has been provided

ii. Autohaemo Injection: in this 4 ml of patient's blood is withdrawn and injected immediately, deep Intra muscular, in gluteal region for stimulation of immune system.

3. Regarding acanthosis nigricans: Grate potatoes, wrap them in a cloth and take out the juice and mix few drops of lemon juice and honey to it and apply it on the affected area and wash off after 20 minutes [Potato has bleaching agents, which helps in reducing/ fading the areas] otherwise take an opinion from Dermatologist regarding management options.

4. Regarding your thyroid and sinus we prepare combine medicine formulation, so if interested I can courier you the result oriented medicine
