My vasculitis occurs in my ankles and feet. Started with red spots and itching. My ankles have become discolored, and not returned to my normal color. The spots keep returning and have traveled upward to my knees. They disappear after 3 to 4 days, but always return. I have no vericose veins or DVT. I was given Mometsone Furoate Ointment 0.1%. I could see no improvement.

Brief Answer:
? Spider Veins from Incompetent hidden Varicose Veins / Under Evaluation

Detailed Answer:
Brief Ans-? Spider Veins from Incompetent hidden Varicose Veins / Under Evaluation

Mostly due to -Varicose Veins with dermatitis from leaking engorged Venules around the ankle

Detailed Answer:
Hi,Dear!! Thanks for the query updates to Me.
Read and reviewed facts and updates of your query, in context to your health worries.

My Opinion-
Dear after reviewing last updated information,You mostly seems to suffer from Spider Veins-Which are engorged Venules, from the undetected Varicose Veins with severe incompetent deep or superficial veins or Varicose veins with hidden DVT(Deep Vein Thrombosis).

Your submission of having discoloured ankles(purplish ankle) with itching, proves that this is due to -Varicose Veins with dermatitis from leaking engorged Venules around the ankle.
The dermatitis and itching with it -is caused from reaction to the blood leaked in to surrounding ankle subcutaneous tissue.

Other causes need to be ruled out by Consults General or Vascular Surgeon, soon.

Do' Suggested-
To control these upgrowing spider veins and discoloured ankle, you need to Consult Vascular Surgeon and get EVLT to correct the reverse vein flow from the incompetent leg veins.

Care-Prior to this surgical treatment-
Get USG with Colour Doppler Study done.
Itching could be controlled with tab loratadine,

but would resolve only after following preventive care-
Rest to the legs, with sleeping in head low position x 2-3 weeks
Start Using VenoStockings.
Avoid long standing job works.
Tab Dafflon- 300 mg x 3 times aday x 3 weeks
Tab NSAIDS-Motrin- 400 mgm x 2-3 times x 10 days
Tab Chymoral forte-1 x 3 times a day x 2-3 weeks
Antibiotis -orally to control any accompanied infection around ankle.
Tab Zincovit 1 / day x 2-weeks

Stop Using any cream, as it would only keep the area wet and prone to more infection in this compromised skin around ankle.
ALERT-
Planned EVLT with sclerotherapy is the only solutions for to avoid recurring spider veins , growing upwards in the legs.
No medicines would correct the Vein defect you have.

Next appointment-
Only after 2 weeks time if the problem increases.

Hope this reply would help you to plan further care to resolve your health issues.
