Question: Should I be worried if my bp spikes in the afternoon and evening. Usually runs about 140/85 in the afternoon and 180/90 in the evening. I am on Coreg twice a day at 3.12 dosage.

Well, blood pressure is a normally fluctuating variable of the human organism.

Allow me to explain that physical activity, an acute or chronic psychological stress, physiological changes in hormonal levels that happen during a day, caffeine-containing beverages, can and will cause these spikes of blood pressure. And, it can happen to anybody, including me. If this is the case, then there is nothing to worry about, as long as the figures are under 140/90 mmHg for the greatest part of the day.

If this is not the case, and your blood pressure figures are found higher than this for the great part of the day, then an increase of carvedilol to 6.25 mg twice a day would be reasonable, also adding a second drug to your therapy. As long as these figures are referred as "spikes", then they are most probably attributed to the above mentioned causes, and no added therapy is needed.
