Question: Dear Doctor,
I had PCOS and after the long treatments and alternate day follicular studies the egg was formed.
And afterwards doctor did one injection for rupturing the egg, after all this treatment finally I got felling of pregnancy and I conformed pregnant by doing blood test and I found the HCG value in first time was 209.6 (on 6th Feb 2015) and doctor told me that this value is low and doctor told me to do the test after 48Hrs then the value becomes 137.0(on 9th Feb), during this period there was an little bleeding was present (like spotting). After two days 12the Feb I felt heavy pain in abdomen and lemon size blood clot was discharged on the same day.
Finally doctor decided that it was a biochemical pregnancy and again doctor advised us to do the blood test then again, we found the HCG value of 60.7. (On 13th Feb)
Now we want to know what we have to do and what is the next course of treatment.
I am very much eager to get pregnant.
Please guide me sir.

Brief Answer:
weight reduction is most important

Detailed Answer:
Hello dear, thanks for the information. I understand your concern.

In my opinion you have started the infertility treatment early. Usually infertility treatment is initiated if the pregnancy doesn't occur even after one year of unprotected intercourse.
You are on correct line of treatment presently. Clomiphene citrate is the drug which causes ovulation in 70% of cases. So clomiphene along with glycomet increases the efficiency. This combination can be tried upto 6 cycles.

If no response with this you can go for intrauterine insemination( IUI) along with above combination.

If no response with clomifene then laparoscopic ovarian drilling of PCOD can be done. Here the cysts are punctured and increases the chances of regular ovulation. This is effective treatment for pcod.

And the last resort is artificial reproductive techniques(ART).
As you are only 25 you might not go upto that extent.PCOD will have good responses to above medicines.
PCOD is routinely seen these days due to unhealthy lifestyle and unhealthy foods.

I suggest you that weight reduction is the first and most important thing in PCOD. Even a 10% loss might make the cycles regular and increases the chance of fertility. According to your height the weight should be 61 kg. So you need to decrease a good number of kgs of weight. Adapt a healthy lifestyle by avoiding junk and high calorie diet. Take low calorie diet and fresh fruits and veggies. Walking or exercise for one hour daily.

These simple measures will help you a lot getting natural pregnancy. Try these along with PCOD treatment.

Avoid stress because stress and anxiety are also the causes for infertility.
Just relax. There is good time ahead.
Hope this satisfies your query.
Feel free to discuss furthur.

Dear Doctor,
Thank you so much very for your detailed explanations.

With regard to the infertility I had mistakenly given as two years.
For your information, for the last two cycles doctor had advised me to take Goodva50mg (From the second day of my cycle.)

For the PCOD, I am taking Medicine for two years.

Finally, I need one clarification.

After the blood clot discharge, next five days I had heavy bleeding (from 13th Feb to 18th Feb- due to Miscarriage.)

Afterwards I had very little blood with white discharge (brownish spotting from 19th Feb to 24th Feb.)

Even now also I had little blood with white discharge (it’s happening every after two or three days)

Please guide me what Medicine we have to take for controlling the little spotting and also please inform me what could be reason.

For your information due to this spotting I have no problem with my normal life.

Once again Thanks for your kind advise.

Brief Answer:
Ultrasound need to be done

Detailed Answer:
Hello dear,I got your query.I understand your concern.
In my opinion it is good to know that you are on infertility treatment since 2 months.
Goodva 50 mg is nothing but clomiphene.
The irregular spotting after miscarriage might suggest incomplete abortion. So I suggest you to get an ultrasound done to see if miscarriage was complete.
If any products are found in uterus they need to be evacuated by minor surgical procedure.
If ultrasound shows normal study then you might need hormones like progesterones to control this irregular spotting. So discuss regarding them with your doctor.
I once again stress on weight reduction to you.And as the miscarriage has happened better give a gap for 3 months and then try for a pregnancy rather than immediately for good results.
Hope this satisfies your query.
Feel free to discuss furthur.
Best regards...Dr srilatha
