Detailed Answer:
Dear

Thanks for contacting Ayurveda with your health concern.

1. low sperm count or poor sperm quality is one cause of infertility in couples but make sure your wife hormonal profile-examination-investigations is/are normal so that treatment is directed to you only.

2. any exposure to environmental toxin,
. occupation [sitting for prolonged hours]
. addiction [alcohol/smoking]
. weight [overweight or normal
. stress [physical/mental/emotional]

3. going by your Semen Reports

. Semen volume is 3 ml [normal is 2-5ml] is OK
. sperm count is 38 million/ml [normal is 50-200 million/ml] so is low
. sperm motility: 70% are non motile which is reverse of normal active motile i.e. 60-80% should be actively motile.

PS. I usually prefer to check two or more semen samples over time to ensure consistency between samples and your doctor might go for scrotal ultrasound and other investigations [if deemed necessary]

4. Speman promotes spermatogenesis by improving the testicular, seminal vesicle and epididymal functions and improves the sperm count and the quality of semen by increasing the LH-FSH producing basophil cells in the pituitary.

~Adverse effects: none reported
~Contraindications: no absolute contraindications
~Special precautions: found safe and well tolerated. However, it is advisable to consult an endocrinologist, if testicular functions do not improve inspite of long term treatment, as a single or adjuvant to other modern drugs.
~Dosage: 1 tablet thrice daily for 4-6 months

5. Do revert back regarding said questionnaire for Ayurveda Treatment and Management.

Brief Answer:
Delayed Seminal Liquefication and Male Infertility

Detailed Answer:
Dear

Thanks for reverting back with your concern and clarifications

1. Since reports of wife are normal thus treatment/management will be addressed as male sexual disorder.
2. since job [IT[ demands prolonged sitting thus make sure to wear boxer undergarments instead of frenchies.

3. Normal semen coagulation takes 1/2 hour following ejaculation, and your test results shows Delayed liquefaction of 3 & 5 hours respectively which means/ may indicate disorders of accessory gland function.

i. To diagnose liquefaction problem your doctor should make sure if there is absence of sperm in the PCT( Post-Coital Test ).
ii. Liquefaction is caused by an enzyme [protease] made in your prostate gland,if your semen is not liquefying quickly enough, it means prostate is not producing this enzyme in sufficient quantities,The fertility clinic will let you know what the necessary treatment is.

4. If you PCT comes normal it is OK to start with Speman [but keep in mind SPECIAL PRECAUTION regarding SPEMAN as stated earlier] and moreover Ayurveda Drug will help you increase sperm count and motility and one can also be a good candidate for AIH [artificial insemination husband]

5. Take: XXXXXXX XXXXXXX fruit,banana,coconut,water melon,pomegranate,raisins, dates,almonds,apple,pears,walnut,urad dal,carrot,apricot,amla,musk melon, bottle gourd,brinjal,potato XXXXXXX milk,milk cream,jaggery mixed curd,butter, onion.

. make sure your intestine are clean [take trifla churan 1gm with lukewarm water at night regularly]
. prior to sleep at night wash your hands upto elbow, feet upto knee and penis with cold water.
. 60 gm red onion pieces
60gm cows ghee
250ml cows milk
Procedure: take the ingredients and cook in cows milk, later when it becomes concentrated, allow it to cool and eat with 10-15 gm mishri for 15 days [morning and evening]
