Dear

Thanks for contacting Ayurveda with your health concern and following points need clarification regarding Ayurveda Management:

1.Were you evaluated by an Orthopedic Surgeon for any structural problem [ruptured or bulging disc] & what was his/her observation? [were any investigations carried out to rule nerve compression]
PS. since there was history of Sciatica

2. I usually enquire about following Risk/aggravating factors from my patient(s) and same is requested from your side:
i. Age - older adults are more susceptible [age is fine]
ii. Anxiety, Depression?
iii. Obesity/overweight [please mention about your weight]
iv. any addiction [Smoking in particular]
v. Strenuous physical exercise or physical work [poor posture]

3. any associated symptoms with back pain like: pain down the legs, urinary incontinence, fecal incontinence, numbness around the genitals, anus, buttocks

4. Do let me know about the medicines which your Doctor has given, so that any drug-drug interaction is avoided.

5. Do revert back for Ayurveda Treatment and Medicines, in the mean time start incorporating following Ayurveda Dietary Regime and lifestyle changes:

i. Avoid: urad dal, curd, fish, jaggery, milk, incompatible and heavy food, suppression of natural urges, anxiety, grief, outdoor activities when sky is covered with clouds, and cold wind.
ii. Take: maize, horse gram, karela, dried ginger, garlic, ajwain, saunf, XXXXXXX marich, saindhav lavan, hing, jeera, old ghee, lukewarm water, castor oil, light walking, sudation, warm clothes, garlic and ginger mixed butter milk.
iii. fry 250 ml ginger juice in 250 ml of til tail, when 250 ml remains [make sure oil doesn't get burn], filter it after cooling and massage.
iv. boil 1 spoon dried ginger in 4 cups of water till 1 cup remains, filter it and add 2 spoon castor oil and drink.

PS.
i. Maintain straight back posture while sitting or standing.
ii. avoid bending forwards and lifting weights.
iii. sleep on hard mattress.
iv. gentle massage
v. back exercises: Leg raising in dorsal recumbent position, push ups, swimming.

Brief Answer:
Back pain Alleviation & Management

Detailed Answer:
Dear

Thanks for reverting back.

1. Since the problem is only during vacations and while staying in hotels thus probable circumstances is/are:

i. Sleeping position:
. If you sleep on your stomach [for prolong time] the spine will develop greater curve than normal which will cause pain and stiffness
. If you sleep on your back [for prolong time] the spine will get flatten and causing the back to be stiff and painful.

PS.
. Best position for you will be sleeping on your side i.e. alternating between right and left side
. Relax the spine completely.
. Do not lie face down.
. Lie on your back with pillow under the knee.
. Lie on the side with a pillow between the knees.

ii. Mattress: If still while staying in hotel and following proper position, the pain and stiffness is there, probably the cause is with the mattress.

PS. make sure to ask for rubber latex mattress instead of spring mattress [or make sure spring mattress in not 3-4 years old]

iii. Management for back pain during vacations:
. make sure your car seat provide adequate back support [since long journey with poor back support can be the initial insult which gets aggravated during stay]
PS. check with specialized cushions/pillows

. maker sure to stretch your hamstrings and back muscles during journey breaks which will improve blood circulation and keep disc, muscle, ligaments and joints active and healthy.

. It is a common practice to substitute your prescription pain medicine with over the counter [OTC] medication since these are often effective in limiting inflammation associated with back pain.
. Boil 100 gm black pepper powder in 300 ml til tail--- filter, cool and massage OR mix dried ginger in castor oil and massage. [you can keep this with you while travelling] and/or alternate Ice with Heat Therapy is another alternative.

Brief Answer:
Personal Preference

Detailed Answer:
Dear

Thanks for reverting back.
1. Since there is no single type of mattress or sleep position which fits/suits all people thus finding a right mattress is personal choice.

2. as explained earlier, under the best sleeping position which completely unloads the stress on the spine and allows for the natural curve of the lower back depends upon individual preference

3. As you said earlier that it is impossible to sleep/find hard bed while travelling/staying so following options can be considered:

i. Since spine wants support thus check with hard bed initially [since it will keep the spine in same body position], if not possible

ii. check with inner spring mattress which can support your weight, but spring within the mattress only last for few years, again one has to ask with the hotel authorities regarding their viability.
PS. back home one should replace them every 3-4 years or make sure to check them regularly.

iii. one can look for latex mattress since latex being a rubber material will hold its shape and support for longer time.

PS.
. as explained repeatedly, the firmness of the mattress will depend on the comfort level of the individual, i.e. neither you want it to be too hard or too soft, bottom line is that you should be able to lie on the bed and feel level.

. It is always advisable for people with back pain to consult with a health care provider for a thorough exam, diagnosis, and treatment program.

. same consideration should go for neck support since good neck support is essential for a good night sleep.
