Suggest Alternative Medication In Place Of Diltiazem

Question: High Blood Pressure . I have Mylodyspasic syndrome being treated with Revlimid and an Epo shot every two weeks. I have a history of many allergies. Negative from Metoprolol , very irritated digestive system. Negative from Bystolic, Valsartan and Norvasc are thick white mucus, sneezing, puffy tongue, blisters on roof of mouth.

I tolerate Diltiazam. 180 mg am, 120 when pressure is over 150. Does not control levels nor the rapid heart rate that comes with pressure. Any suggestions for another drug to try? Man thanks

I have absolutely no excess fluid and the Revlimid dehydrates the body. My heart is strong. I need to hydrate frequently but. Excess is never evident Only blood pressure immediate problem. Yesterday's numbers from awakening till night are as follows : 177/97 p98, 185/87 p103, 160/91 p84, 175/89 p 95, 165/82 p101.

Will the diuretic not add to the dehydration? I will discuss this with my physician next week. I appreciate your suggestion and I shall pursue it. I wonder if you might also have a suggestion for an allergy treatment that does not adversely affect blood pressure.
Just looked up hydrochlorthiazid and I neglected to tell you that some years ago I went into anaphylaxis after taking Gantrasin, penicillin and sulfa. So I avoid them. Any other thoughts?

I read your query and understood your concerns.

Since you are reporting to have allergic reaction from two many pills I would recommend to switch from diltiazem to Carvedilol.

Carvedilol is a betablockers who has also alfa blockers activities. It's effective in lowering BP and in controlling HR.

Also I would recommend to take only medical treatment for myelo dysplasia during changing your therapy.

If the new treatment is effective in controlling BP without allergic reaction you don't need any other drugs.

Otherwise you may add hydrochlorthyazide after two weeks (if Carvedilol was well tolerated).

Although you are dehydrated you may use hydrochlorthyazide in low doses (12.5 mg) to further lower BP to optimal values. In case of mild to moderate dehydration you may take gatorade juice (sport drinks). You should stop taking hydrochlorthyazide in case of severe dehydration.
