Question: Subject: Weight loss & Stretch marks:

My wife has reduced her weight but now facing difficulty for belly fat and loose skin.

Age: 47 years
Nationality: Pakistani
Lifestyle: Housewife, 2 kids
Weight loss: From 73 kg to 58 kg in one year
Method: Using Elliptical for 1 hour, diet control

Presently, her statistics are:
Height: 62", 157 cm
Bust 35", 89 cm
Waist 31", 79 cm
High hip 35", 89 cm
Hip 36", 91 cm

She wants her waist to be 60-65 cm which is now 79 cm but belly fat is a big problem.

Overall, she has reduced weight significantly but belly fat is reducing very very slowly. And after one year of efforts, she's frustrated with the belly fat problem.

Also her skin is getting too loose especially on breast.

Please advise.

Detailed answer below.

Detailed Answer:
Hi Amerabdullatif2013,
Thanks for using Healthcare magic.I understand your concern.
I have read about all the details you had given , about your wife's efforts &the results she has...& it seems to be satisfactory.
While loosing weight face is the first one to show changes & abdomen shows results the last...I think she should not worry & continue with her line of management, she would be satisfied over some time.
I find few facets in her weight management programme...I would like to share it with you.
1. For overall weight loss...only diet&exercises are not the answers.
Has she checked her blood sugar, thyroid function ? High blood sugar&low thyroid stimulating hormone make difference. These should be checked & managed as per need , along with her present treatment .
Similarly genetics also plays a part.
2. About loss of abdominal girth : -
* She should concentrate on Abdominal exercises more...specially 'Yog' exercises (they involve mind also while doing physical activity..this is found more effective)
*Clearing of bowels & digestive health should be considered.
* About the Elliptical...she has to follow advise of the expert strictly.
3. Management of wrinkles : When there is weight gain &it remains over a long time...skin gets stretched&it looses it's elasticity..so after vigorous weight loss...skin gets wrinkled. Similarly with vigorous diet control., the elastin/collagen in skin gets damaged....&skin looses its ability to retract.
* To manage this ...I would suggest-
-Supplimenting Vitamin C, omega3fatty acids in diet.
-facial exercises for building facial muscles &protein suppliments(after discussing with her dietitian)
-Avoid exposure to sun
-Stop smoking.
- regular body massage /firming cremes.
She has to discuss these points with her expert & apply them to suit her health status.
I hope that answers your query.Any follow up query is welcome.
Thanks.

Dear Dr. XXXXXXX

Thank you for your advice.

You asked to consult an expert but I made the payment for a specialist Dietician & Nutritionist so kindly refer my case to your specialist colleague.

I have attached two compete body analysis of my wife:

1. XXXXXXX 2019, weight was 69.1 kg
2. March 2020, weight was 58.6 kg

She is now very near the target weight of 55 kg but the balance need to be corrected between belly fat and overall body weight.

Her blood sugar and thyroid are normal. She does not have any other medical problems except tension or depression which is a common thing now.

I want to share her diet plan with your expert Dietician to know what improvement can be done.

Daily Diet Plan:
7:30 am
Two eggs without bread
OR Porridge with 2-3 tbsp mixed nuts, no sugar
Plus 250 ml low fat milk boiled with tea and cardamom.

12:30 pm
Green salad, Regular home cooked meal
No bread or roti

5:00 pm
250 ml low fat milk boiled with tea and cardamom
Plus 2 tbsp mixed nuts

7:30 pm
Green salad, Regular home cooked meal
No bread or roti

All home cooked meals are with olive oil and usually follow below pattern for 7 days of the week.

1. Chicken Biryani
2. Baked or grilled chicken
3. Chicken or Mutton curry
4. Vegetable curry
5. Lentils or beans
6. Haleem or lentils or vegetables
7. Meat Pasta or Chicken Pizza

Important notes:

1. Every second day, she ends up with some sort of binge eating like fried items, chocolate, biscuits, ice cream or any other dessert.

2. She takes Omega 3 tablet daily. We try to eat salmon but only once a month.

3. She takes green apple + beetroot fresh juice 3-4 times a week without sugar.

4. She exercises one hour on elliptical and another 30 minutes cardio on daily basis.

5. Water intake is less but tries to manage 1.5 - 2 litres a day.

6. Her carb intake is less as she doesn't take any bread or roti on daily basis. Occasionally she takes it like rice, pasta or with specially prepared meals.

7. Her body statistics are:

Bust 35", 89 cm
Waist 31", 79 cm
High hip 35", 89 cm
Hip 36", 91 cm

She wants her waist to be around 65 cm.

My question is what more should we do to help her reduce the belly fat?

Am I supposed to do something?

Or will you refer to the specialist yourself as you wrote?
