Normally, the sinuses are basically just air-filled cavities, while on CT-scan they tend to appear black/dark. But if it appears as grayish/whitish in color in the CT, it's termed as Opaque. It may or may not be related to scar tissue (although due to chronic inflammatory changes, some deposition of scar tissues can be possible).

The maxillary sinuses are the butterfly-shaped structure just behind our nose on both sides. The CT report of 02/06/2015 is predominantly indicating features of chronic Maxillary sinusitis (Right side) in this scenario.

I can understand your concerns. Please upload the 2018's CT-report also so that I can compare them both, and let you know more precisely.

As I said, some scar tissue deposition is possible over a period of time (3yrs time period is significant), I can understand the ENT's concern too. Let me have a look at them both tomorrow, I'll let you know the possibilities/next best alternatives thereafter...
