Question: I've always had the ability to ejaculate large amounts of fluid when I masturbate, and I've always masturbated daily, often several times a day. A year ago I started to have trouble peeing, and eventually I started to pee blood in my urine. I went to the doctor who sent me to a specialist. The specialist found a growth in my bladder. He said some of my prostrate gland had grown into my bladder and it was blocking the flow of urine, so the excess prostrate had to be removed. I had an operation to remove the growth a year ago and fortunately the growth was not cancer. The operation was done without making an incision, it was done by pushing a tube through my penis up into my bladder and using something on the end of the tube to remove the part of my prostrate that was blocking the bladder. I think the surgeon also reduced the total size of my prostate at the same time. After I healed up from the operation there was only one lasting side effect. I still masturbate almost every day, but no fluid is ever ejaculated. I can feel the orgasm the same way I always have, but nothing comes out of my penis, except for a very small amount of pre-cum which is produced while I'm masturbating. My surgeon says he can't do anything about this because the orgasm is entirely in my mind, and nothing is produced. But, that does not make any sense to me because I was masturbating and producing plenty of fluid right up to the time I saw blood in my urine and went to the doctor. I believe the fluid is still being produced and trapped somewhere in my body, because a passage to the penis is blocked. I don't have any pain, but I worry that if I don't give up masturbation, then eventually the buildup of this trapped fluid will cause a problem. Is there an operation that will fix this? Should I stop masturbating, even though the urge is still there every day (often several times a day)? If I don't do anything about this will the buildup of cum inside of me over time eventually cause something to rupture from the pressure?

The surgery you underwent causes a condition known by the name of retrograde ejaculation in 90 % of the patients. You masturbate or have intercourse and experience orgasm but no ejaculate comes out because everything goes into bladder and is passed out with urine.

It is a very well known side effect and does not affect your health or sexual power in anyway.

You can continue masturbation.

You can consult the Doctor to confirm the same.

My doctor did mention that this problem is due to retrograde ejaculation into the bladder. However, I have difficulty believing that explanation
for the following reason: Before the operation, I had the urge to urinate after sexual intercourse or masturbation, and there was always a head of thick white foam in the toilet floating on the urine. I have no medical training, but it seems obvious to me that this white foam was caused by a residue of semen being flushed out of my urinary track during post coital urination. This theory is reinforced by the fact that I do not recall the presence of this heavy white foam in my urine prior to puberty. Logically speaking, if the semen is going into my bladder, then one would expect to see white foam in the toilet from the retrograde ejaculate being flushed out of the bladder. But, I do not see any white foam in the toilet after intercourse now, which makes me suspicious that the ejaculate is going
backward into my bladder. The other possibilities are: 1.) The duct which delivers the ejaculate to the urethra was sealed off during the operation. 2.) My orgasms are imaginary and no fluid is being produced, which is what my doctor said after I presented the missing foam argument to him. I reject possibility number two for the following reason. If my orgasms are imaginary, then why do I have the frequent urge to masturbate. Surely, that urge results from pressure due to the build up of ejaculate fluid in a gland, just as the urge to urinate results from the build up of pressure from urine in the bladder. You may ask why any of this is important. Because, without the ability to ejaculate semen I no longer have the ability to produce children. And, I would like to have a child before I die. If the cause of this problem is a blocked duct, then there is a possibility that the duct could be reopened by surgery, restoring my ability to have children. My followup question is, if your initial answer was correct, then why don't I see foam from the retrograde ejaculation when I empty my bladder immediately after intercourse?

The foam which you used to occur in the past was probably due to orthophosphates in the urine rather than semen. The orthophosphates usually results when urine is incompletely emptied and that has corrected after this surgery.

Since you had normal ejaculation earlier , it is very likely that you are currently experiencing retrograde ejaculation. A routine urine analysis - post coital urine sample is tested for presence of abnormal quantities of semen may be done to confirm this condition.

Retrograde ejaculation can be treated using medical drugs. As far as parenting a child is concerned, you can still have children. The urine after ejaculation is collected within 2 hrs of intercourse with prior treatment with sodium bicarbonate. The sperms are seperated from semen and are injected into your partner by intrauterine insemination.

Discuss about the tests and treatment with your urologist.
